A qualitative exploration of the use of telehealth for opioid treatment: Implications for nurse‐managed care

Abstract

Aim

To characterise experiences with telehealth for Medications for Opioid Use Disorder (MOUD) services among patients, prescribers, nurses and substance use counsellors to inform future best practices.

Design

We engaged a qualitative descriptive study design.

Methods

Semi-structured interviews were conducted with prescribers (nurse practitioners and physicians, n = 20), nurses and substance use counsellors (n = 7), and patients (n = 20) between June and September 2021. Interviews were verbatim transcribed. Thematic analysis was conducted using a qualitative descriptive method.

Results

Among both providers and patients, four themes were identified: (1) Difficulties with telehealth connection (2) Flexibility in follow-up and retention, (3) Policy changes that enabled expanded care, (4) Path forward with telehealth. Two additional findings emerged from provider interviews: (1) Expansion of nurse-managed office-based opioid treatment, and (2) Novel methods to engage patients.

Conclusions

Patients and providers continued to view telehealth as an acceptable means for delivery and management of MOUD, particularly when utilised in a hybrid manner between in-person visits. Nurse-managed care for this service was evident as nurses extended the breadth of services offered and utilised novel methods such as text messages and management of ‘call-in’ lines to engage patients.

Implications for the profession and/or patient care

Use of telehealth for MOUD should be incorporated into practice settings to reach patients in a flexible manner. Nurses in particular can use this medium to extend office-based opioid treatment by conducting assessments and expanding capacity for other wrap-around services.

Impact

We identify recommendations for best practices in the use of telehealth for opioid use disorder management and highlight the value of nurse-managed care.

Antibiotic use among hospitalised patients in Sierra Leone: a national point prevalence survey using the WHO survey methodology

Objective

Inappropriate use of antibiotics is a major driver of antibiotic resistance. A few studies conducted in Africa have documented that about half of hospitalised patients who receive antibiotics should not have received them. A few hospital-based studies that have been conducted in Sierra Leone have documented a high usage of antibiotics in hospitals. Therefore, we conducted a nationwide point prevalence survey on antibiotic use among hospitalised patients in Sierra Leone.

Design

We conducted a hospital-based, cross-sectional survey on the use of antibiotics using the WHO point prevalence survey methodology.

Setting

The study was conducted in 26 public and private hospitals that are providing inpatient healthcare services.

Participants

All patients admitted to paediatric and adult inpatient wards before or at 08:00 on the survey date were enrolled.

Outcome measures

Prevalence of antibiotic use, antibiotics Access, Watch and Reserve (AWaRe) categorisation, indication for antibiotic use prevalence and proportion of bacteria culture done.

Results

Of the 1198 patient records reviewed, 883 (73.7%, 95% CI 71.1% to 76.2%) were on antibiotics. Antibiotic use was highest in the paediatric wards (306, 85.7%), followed by medical wards (158, 71.2%), surgical wards (146, 69.5%), mixed wards (97, 68.8%) and lowest in the obstetrics and gynaecology wards (176, 65.7%). The most widely prescribed antibiotics were metronidazole (404, 22.2%), ceftriaxone (373, 20.5%), ampicillin (337, 18.5%), gentamicin (221, 12.1%) and amoxicillin (90, 5.0%). Blood culture was only done for one patient and antibiotic treatments were given empirically. The most common indication for antibiotic use was community-acquired infection (484, 51.9%) followed by surgical prophylaxis (222, 23.8%).

Conclusion

There was high usage of antibiotics in hospitals in Sierra Leone as the majority of patients admitted received an antibiotic. This has the potential to increase the burden of antibiotic resistance in the country. We, therefore, recommend the establishment of hospital antimicrobial stewardship programmes according to the WHO core components.

Targeted solutions to increase dolutegravir coverage, viral load testing coverage, and viral suppression among children living with HIV in Togo: An analysis of routine facility data

Background

According to UNAIDS, Togo halved AIDS-related deaths among children ages 0–14 from 2010 to 2020. However, available data show low dolutegravir (DTG)-containing antiretroviral therapy (ART) coverage and low viral load suppression (VLS) among children living with HIV (CLHIV). We analyzed routine facility data before and after implementation of root-cause-based solutions for improving DTG coverage, viral load (VL) testing coverage, and VLS among CLHIV.

Description

We analyzed routine data for CLHIV ≤14 years from October 2019 through September 2022. We assessed proportion of CLHIV on ART receiving DTG, VL testing coverage (CLHIV on ART with documented VL test result), and VLS (CLHIV with documented VL test result of Results

From baseline (October 2019–September 2020) to endline (October 2021–September 2022), increases were observed for DTG coverage (52% to 71%), VL testing coverage (48% to 90%), and VLS (64% to 82%). Age-disaggregated data showed positive trends.

Conclusions

Root-cause-based solutions and granular data use increased DTG coverage, resulting in increased VL testing and VLS among CLHIV. These interventions should be scaled and become the national standard of care.

Lambs supplemented with Amazonian oilseed co-products: Meat quality and fatty acid profile

The Amazon has a wide variety of oilseeds that generate a huge amount of co-products with potential for use in animal nutrition. The objective was to use alternative resources (oilseed cakes) in the feeding of lambs to assign a sustainable destination to this biomass, and evaluate its influence on the quality and fatty acid (FA) profile of the meat. Twenty-four lambs, male, castrated, crossbred Dorper × Santa Inês, weighing 30 ± 1.3 kg of initial body weight, were distributed in a completely randomized design in 4 treatments (diets) with six replications (animals). The control diet (Control) contained corn and soybean meal as main ingredients, which were partially replaced in the other diets by cupuassu cake diet (Cup), palm kernel cake diet (Palm) and tucuma cake diet (Tuc). The inclusion of Amazon cakes influences the lipid (P = 0.02) and protein (P longissimus lumborum); reduces cooking losses (P 0.05). The inclusion of oilseed cakes influences the chemical composition, physical parameters, composition and fatty acid profile of the meat, but does not influence the indicators of atherogenicity, thrombogenicity and cholesterolemia.

Measuring the success of programmes of care for people living with dementia: a protocol for consensus building with consumers to develop a set of Core Outcome Measures for Improving Care (COM-IC)

Introduction

The Core Outcome Measures for Improving Care (COM-IC) project aims to deliver practical recommendations on the selection and implementation of a suite of core outcomes to measure the effectiveness of interventions for dementia care.

Methods and analysis

COM-IC embeds a participatory action approach to using the Alignment–Harmonisation–Results framework for measuring dementia care in Australia. Using this framework, suitable core outcome measures will be identified, analysed, implemented and audited. The methods for analysing each stage will be codesigned with stakeholders, through the conduit of a Stakeholder Reference Group including people living with dementia, formal and informal carers, aged care industry representatives, researchers, clinicians and policy actors. The codesigned evaluation methods consider two key factors: feasibility and acceptability. These considerations will be tested during a 6-month feasibility study embedded in aged care industry partner organisations.

Ethics and dissemination

COM-IC has received ethical approval from The University of Queensland (HREC 2021/HE001932). Results will be disseminated through networks established over the project, and in accordance with both the publication schedule and requests from the Stakeholder Reference Group. Full access to publications and reports will be made available through UQ eSpace (https://espace.library.uq.edu.au/), an open access repository hosted by The University of Queensland.

Protocol for a prospective cohort study on the feasibility of application of nutritional ultrasound in the diagnosis and follow-up of patients with nutritional risk at hospital discharge: study on body composition and function (DRECO)

Introduction

Nutritional ultrasound (US) is an emerging technique in clinical nutrition for the morphological and structural study of muscle mass. Currently, all definitions of malnutrition include the measurement of muscle mass; however, there is no single way to assess it. It is necessary to develop new techniques to identify muscle involvement in malnutrition that are valid, standardised, reliable, accurate and profitable.

Objective

To value the new muscle US techniques aimed to measure muscle and functional status, to make a more accurate diagnosis and a better prediction of complications and morbidity and mortality in patients at nutritional risk. Primary outcome: to assess the feasibility of US or muscle US techniques in both nutritional diagnosis and follow-up in a nutritional intervention programme.

Methods and analysis

Disease-Related caloric-protein malnutrition EChOgraphy (DRECO) is a prospective, multicentre (25 Spanish hospitals), uncontrolled clinical study in standard clinical practice to value the usefulness of nutritional US (muscle US) in the nutritional diagnosis and follow-up, over 3–6 months, after standard nutritional clinical practice intervention and physical activity, to control their disease-related malnutrition. 1000 patients are expected to be included in.

Discussion

This study will standardise nutritional US measures. It will validate and define specific cut-off values for nutritional US and correlate it with already well-known nutritional tools such as Subjective Global Assessment or Global Leadership Initiative on Malnutrition criteria. Thus, muscle US will become not only a tool to diagnose malnutrition, but it will also be integrated in the daily practice to evaluate nutritional interventions.

Ethics and dissemination

All DRECO study materials have been approved by each of the IRB/IEC of all the sites enrolled (either approval of the own IRB/IEC or validating the approval of the IRB/IEC of another hospital). The study has been registered with ClinicalTrials.gov, on 27 June 2022. The results from this study will be presented at scientific conferences and in peer-reviewed scientific journals.

Exploring fear of cancer recurrence and related factors among breast cancer patients: A cross‐sectional study

Abstract

Aims

Fear of cancer recurrence (FCR) is a multifaceted concept influenced by individual characteristics, social support, psychological factors. This study aims to identify distinct FCR profiles among breast cancer patients and explore the associated variables with these patterns.

Design

A cross-sectional study was conducted from April 2022 to March 2023.

Methods

A convenience sample of 339 patients completed a questionnaire that assessed general and disease-related data, including the Fear of Progression Questionnaire-Short Form, Social Support Rating Scale, Medical Coping Modes Questionnaire. Statistical analysis involved latent profile analysis (LPA) and multinomial logistic regression.

Results

Three latent patterns of FCR were found: the low fear (28.9%), the moderate fear (51.3%), and the high fear (18.0%). The study identified the social support, family monthly income, employment status, utilization of confrontation coping mode and avoidance coping mode, as factors that impacted the FCR.

Conclusions

Social support, family monthly income, employment status, and medical coping modes have been found to impact the FCR among newly diagnosed breast cancer patients. Healthcare professionals should focus on addressing FCR at diagnosis and implement effective interventions, such as promoting social support and encouraging adaptive coping, to alleviate this concern.

Impact

Urgently addressing the FCR in Chinese breast cancer patients is imperative due to its profound influence on their holistic health. Through advanced LPA, we categorized the FCR progression, highlighting risks. These findings have implications for healthcare strategies, offering new insights to manage the FCR and improve patient well-being. Our study adds a fresh perspective to the factors underlying the FCR in breast cancer patients, contributing to the broader comprehension and management of this complex survivorship issue.

Laser and radiofrequency for treating genitourinary syndrome of menopause in breast cancer survivors: a systematic review and meta-analysis protocol

Introduction

Breast cancer survivors (BCSs) experience more severe symptoms of genitourinary syndrome of menopause (GSM) than healthy postmenopausal women. As hormonal therapy with oestrogen should be avoided in BCSs, finding an effective and safe therapy to address vaginal symptoms and sexual dysfunction is urgently needed. Physical methods may be promising alternatives for the specificities of this group of women. This review aims to evaluate the efficacy and safety of physical methods (laser and radiofrequency) for treating GSM in BCSs.

Methods and analysis

The PubMed, Embase, Web of Science, SciELO, LILACS, Scopus, Cochrane Central Register of Controlled Trials and ClinicalTrials.gov databases will be searched. A search strategy was developed to retrieve clinical trials that evaluate the efficacy and safety of any physical method (laser or radiofrequency) used for GSM in BCSs. No date or language restrictions will be imposed. Two authors will independently select studies by title, abstract and full text to meet the inclusion criteria. Data will be extracted, and the risk of bias will be evaluated using the Cochrane risk-of-bias tool (RoB 2). Review Manager 5.4.1 will be used for data synthesis. The Grading of Recommendations, Assessment, Development and Evaluation will be used to assess the strength of the evidence.

Ethics and dissemination

This study reviews the published data; thus, obtaining ethical approval is unnecessary. The findings of this systematic review will be published in a peer-reviewed journal.

Career optimism and job‐related well‐being of Australian nurses during COVID‐19: A cross‐sectional study

Abstract

Background

Australia has been confronted with a severe nursing deficit, making it difficult to maintain a strong healthcare workforce capable of meeting the mounting demands of healthcare organizations.

Aims

This study aimed to understand how personal and organizational resources influence career optimism and job-related affective well-being of Australian nurses during a pandemic using the Conservation of Resource Theory.

Methods

A cross-sectional online survey was emailed to 123 Australian nurses from January to February 2021. The survey consisted of self-reported measures, including mindfulness, career optimism, job-related affective well-being, personal and job resources measures, and the Dirty Dozen scale. Correlations, independent sample t-test, and a series of hierarchical regressions were conducted on the cross-sectional data with SPSS Version 27. The STROBE checklist was used to report the results.

Results

Findings suggested that mindfulness, perceived supervisor support, and job autonomy were significant predictors of job-related well-being, whereas mindfulness, perceived supervisor support, and opportunities for professional growth contributed more to career optimism of nurses during a health crisis. Male nurses in this study reported significantly higher mindfulness, career optimism, and job-related well-being levels than female nurses.

Linking Evidence to Action

Developing mindfulness among nurses, allocating organizational resources to facilitate more supervisor support, and providing job autonomy may enhance career optimism and job-related well-being of nursing staff who work in disruptive and high-demand work environments such as those experienced during the COVID-19 health crisis. Supervisors should also facilitate and encourage nurses to reflect and be mindful of their behaviors with their peers and patients which can help to reduce exploitative or arrogant behaviors in the workplace.

Biografía de la enfermera brasileña Edith de Magalhães Fraenkel y su iconografía fotográfica (1942-1955)

Introducción: El artículo presenta la biografía profesional de un gran líder de la enfermería brasileña, narrada a partir de fotografías y documentos de la época. Objetivo: describir la biografía de la enfermera Edith de Magalhães Fraenkel y analizar el contenido de su iconografía fotográfica, de 1942 a 1955. Metodología: Se trata de una investigación cualitativa, histórico-social y biográfica, guiada por la metodología de análisis e interpretación de fuentes fotográficas de Boris Kossoy.
Resultados: Edith de Magalhães Fraenkel fue una figura destacada de la enfermería brasileña y paulista. Tuvo un desempeño notable en la atención, docencia y gestión de escuela de enfermería y
luchó incansablemente por promover el desarrollo de la profesión en Sudamérica. Son notables las
innovaciones curriculares implementadas en el curso de enfermería durante su mandato como primera directora de la Escuela de Enfermería de la Universidad de São Paulo. Ella promovió la matrícula de hombres y mujeres, blancos y negros, de diferentes clases sociales y de nacionalidad brasileña y extranjera. Conclusión: A través de la dirección de Edith, la Escuela de Enfermería de la Universidad de São Paulo se convirtió en pionera en promover una cultura de igualdad de género y
valorar la diversidad sociocultural entre los estudiantes.

Biografia de João Mauro Moraes, primeiro homem diplomado enfermeiro em Minas Gerais, Brasil

Objetivos: resgatar e refletir sobre a biografia de João Mauro Moraes, primeiro homem diplomado como enfermeiro em Minas Gerais, Brasil. Metodologia: trata-se de pesquisa histórica e biográfica. Os dados foram coletados de artigos científicos, notícias de jornais, livros, documentos e fotografias. A análise seguiu o Método Interpretativo Narrativo Biográfico. Resultados: o artigo apresenta a história de vida de João Mauro (1936-2017) e sua atuação como atendente de enfermagem no Exército, auxiliar de enfermagem, enfermeiro e gerente de enfermagem do Serviço de Atendimento Médico Domiciliar de Urgência de Itajubá. Discussão: João Mauro fez uma escolha consciente pela enfermagem, sendo respeitado e valorizado socialmente. Sua atuação profissional denota compromisso ético, seriedade, firmeza e respeito à dignidade humana. De sua trajetória profissional fez-se uma reflexão sobre a presença masculina na enfermagem, uma profissão para mulheres e homens. Considerações: João Mauro foi um importante personagem da história da enfermagem, do interior do Brasil, na segunda metade do século XX. Seu pioneirismo rompeu com a hegemonia feminina na formação profissional de enfermagem em Minas Gerais. Seu protagonismo contribuiu para a inserção de homens no curso de enfermagem de uma instituição confessional católica, num tempo em que era mínimo o número de enfermeiros diplomados no país.

Factores de riesgo y estilos de vida en adolescentes asociados al cáncer en la adultez: una revisión de alcance

Objetivo: Identificar la evidencia científica relacionada con los factores de riesgo y los estilos de vida en adolescentes asociados a la ocurrencia de cáncer pulmonar, cervicouterino, gastrointestinal, de piel y de mama en la adultez. Metodología: Revisión de alcance basada en la metodología del Joanna Briggs Institute (JBI), se exploraron cuatro bases de datos, a los artículos seleccionados se les realizó análisis, extracción y síntesis de datos. Resultados: Se incluyeron 33 artículos, los factores de riesgo identificados fueron antecedentes familiares, sexo, tabaco, alcohol, sobrepeso, bajo peso, exposición al sol, infección por virus del papiloma humano (VPH), desconocimiento sobre medidas de autocuidado, bajo consumo de frutas y verduras, consumo de carnes rojas y de grasas, bajo nivel socioeconómico, bajo nivel de escolaridad y sedentarismo; y, como estilos de vida: actividad física, uso del condón, seguimiento y detección de su situación de salud, vacuna contra el virus del papiloma humano (VPH), alimentación saludable y protección solar. Conclusión: Los factores de riesgos encontrados en la revisión de alcance son numerosos, según cada tipo de cáncer, existen factores modificables que desde la adolescencia se pueden poner en práctica como estilos de vida en sentido individual, familiar y escolar.

Repercusiones de la pandemia por el nuevo coronavirus para la salud de los profesionales de enfermería

Objetivo: Analizar las repercusiones de la pandemia por el nuevo coronavirus en la salud de los profesionales de enfermería. Método: Investigación cuantitativa, descriptiva, realizada en un hospital universitario de Salvador, Bahía, Brasil, entre septiembre y octubre de 2020. La recolección de datos se realizó a través de un cuestionario electrónico estructurado, que fue respondido por 209 profesionales de enfermería de la institución. El proyecto fue aprobado por un Comité de Ética en Investigación (nº CAAE 36841720.0.0000.0049). Resultados: La mayoría de los profesionales relató insomnio o dormir menos de lo necesario (62,67%); mostró aumento del deseo de comer (46,89%) o pérdida del apetito (16,75%); tuvo pérdidas en actividades de ocio (97,6%); relataron una disminución en la práctica de actividad física (91,87%); mostró alteraciones en el estado de ánimo (83,75%); reportó algún grado de ansiedad (89,95%); y tenía diagnóstico de covid-19 (28,23%). Conclusión: Los resultados indican las repercusiones que trajo la pandemia para la salud física y mental de los profesionales de enfermería.

Experiencia en la atención al paciente con Covid-19: de las reacciones a lo nuevo a la atención sensible

Objetivo principal: Aprehender las experiencias de los profesionales de enfermería en el cuidado de pacientes con covid-19. Metodología: Investigación cualitativa realizada con 17 profesionales de enfermería en un hospital universitario de Salvador, Bahía, Brasil. La información fue recolectada a través de entrevistas semiestructuradas y analizada con el apoyo del software IRAMUTEQ. Resultados principales: La experiencia de las prácticas fue descrita como difícil, aterradora, complicada, causando miedo, aprensión y estrés. Surgió el énfasis en las medidas de autoprotección y como medio para superar el aislamiento, los profesionales utilizaron estrategias para favorecer la aproximación y la comunicación entre los pacientes y el equipo, evidenciando así un cuidado sensible a las necesidades del otro. Conclusión principal: Las reacciones indican que los profesionales desarrollaron estados emocionales negativos; adaptaron el servicio; y el fortalecimiento del autocuidado mediante la exposición al riesgo de contaminación y utilizaron estrategias de atención permeadas por la sensibilidad.

Prevalencia de los cambios dermatológicos en personas diabéticas con ulceración del pie

Objetivo principal: Estimar la prevalencia de alteraciones dermatológicas en personas diabéticas con ulceración del pie atendidas en un centro de referencia en diabetes en Bahía. Metodología: Estudio transversal y retrospectivo. La población del estudio consistió en 133 pacientes tratados en 2017 y la recopilación de datos tuvo lugar entre agosto y octubre de 2018. Para el análisis de datos se utilizaron estadísticas descriptivas realizadas utilizando el software estadístico Statistical Package for the Social Sciences (SPSS 16.0). Resultados principales: El cambio más frecuente fue la callosidad (48,1%) seguido de xeroderma (47,4%), micosis interdigital (18%), onicomicosis (13,5%), fisuras (6,8%). Entre el momento del diagnóstico de diabetes y fisuras se encontraron asociaciones entre el momento del diagnóstico de diabetes y fisuras (p= 0,019), úlcera neuropática y xeroderma (p=0,015), úlcera infectada y callos o callos (p=0,010). La onicomicosis y el xeroderma eran más frecuentes en los hombres, mientras que los callos en las mujeres. Conclusión principal: En esta investigación se encontraron nuevas asociaciones. Se observó que la salud de los pies es una brecha en la literatura y este estudio tiene como objetivo ampliar el conocimiento sobre este tema, sirviendo para guiar los comportamientos de atención al paciente en los servicios y calificar la atención médica de las personas diabéticas.

Facilidades y dificultades de los técnicos de enfermería para realización del electrocardiograma después del entrenamiento en servi-cio

Objetivo: describir las facilidades y dificultades de los técnicos de enfermería para realización del electrocardiograma (ECG) después del entrenamiento en servicio. Métodos: investigación cualitativa, realizada en una unidad cardiovascular de un Hospital Universitario de Salvador - Bahía, Brasil, con 10 técnicos de enfermería. Resultados: el análisis permitió identificar tres categorías: formación como facilitador de la práctica asistencial; uso adecuado de ECG en la rutina de servicio después del entrenamiento; escasez de recursos materiales necesarios para realizar el ECG. Los resultados mostraron que además de los beneficios para los usuarios del servicio, el curso de ECG también puede ofrecer al profesional algunas ventajas, como un mayor conocimiento y un aprendizaje favorecido. Conclusión: las facilidades fueron descritas como ampliación del conocimiento y favoreciendo el aprendizaje de las técnicas correctas para la realización del procedimento, conduciendo a una disminución de errores y el uso adecuado del dispositivo. Las dificultades estaban relacionadas con la escasez de materiales y las precarias condiciones de algunos dispositivos.

Notificaciones de lesiones de piel en un Hospital Universitario

Objetivo: Analizar las notificaciones de lesiones de piel en un hospital universitario. Método: Estudio descriptivo, realizado en un hospital universitario, localizado en Salvador, Bahia, Brasil, con datos secundarios de notificaciones de incidentes. Fueron investigadas características sociodemográficas de los pacientes con lesiones de piel y características de las lesiones notificas en los años 2016 y 2017. Los datos fueron recolectados entre los meses de septiembre 2018 y mayo 2019 y el estudio respetó los principios éticos de investigación. Resultados: Fueron notificados 96 casos de lesión de piel. La mayoría fue adquirida en la hospitalización (89,58%), siendo frecuente la lesión por presión (77,08%), en la región sacra o glútea (61,46%), con pérdida de piel (54,17%), en grado leve (89,58%) y que prolongaron la hospitalización (7,29%). Conclusión: A pesar de ser realizada evaluación del riesgo para lesión, en más de la mitad de las notificaciones se observa que el tipo más prevalente fue la lesión por presión.

Evidencias bibliográficas sobre riesgos laborales en los pescadores artesanales: revisión integrativa de la literatura

Objetivo: Identificar en la producción científica los tipos de riesgos para enfermidades y agravos para acidentes em trabajadores de la pesca artesanal. Metodología: Revisión integrativa, realizada entre enero y marzo de 2018, por medio de la combinación de los descriptores "Occupational Health" y "Fisheries", en las bases de datos, Medline, LILACS, Scopus, SciELO e internet. Se encontraron 123 artículos. Después de la lectura y evaluación de los mismos, se obtuvo un resultado final para el análisis de 16 artículos. Resultados principales: La mayoría de los estudios se indexaron en la base de datos Medline (56,3%), en el idioma inglés (43,8), con un nivel de evidencia VI (100%). Se constató que los trabajadores de la pesca artesanal están expuestos a riesgos ergonómicos, químicos, físicos y de accidentes. Conclusión: Se constató carencia de publicaciones en el área de la pesca artesanal, inexistencia de publicaciones científicas realizadas por la enfermería y brechas en el conocimiento.
