Specific nanoprobe design for MRI: Targeting laminin in the blood-brain barrier to follow alteration due to neuroinflammation

Chronic neuroinflammation is characterized by increased blood-brain barrier (BBB) permeability, leading to molecular changes in the central nervous system that can be explored with biomarkers of active neuroinflammatory processes. Magnetic resonance imaging (MRI) has contributed to detecting lesions and permeability of the BBB. Ultra-small superparamagnetic particles of iron oxide (USPIO) are used as contrast agents to improve MRI observations. Therefore, we validate the interaction of peptide-88 with laminin, vectorized on USPIO, to explore BBB molecular alterations occurring during neuroinflammation as a potential tool for use in MRI. The specific labeling of NPS-P88 was verified in endothelial cells (hCMEC/D3) and astrocytes (T98G) under inflammation induced by interleukin 1β (IL-1β) for 3 and 24 hours. IL-1β for 3 hours in hCMEC/D3 cells increased their co-localization with NPS-P88, compared with controls. At 24 hours, no significant differences were observed between groups. In T98G cells, NPS-P88 showed similar nonspecific labeling among treatments. These results indicate that NPS-P88 has a higher affinity towards brain endothelial cells than astrocytes under inflammation. This affinity decreases over time with reduced laminin expression. In vivo results suggest that following a 30-minute post-injection, there is an increased presence of NPS-P88 in the blood and brain, diminishing over time. Lastly, EAE animals displayed a significant accumulation of NPS-P88 in MRI, primarily in the cortex, attributed to inflammation and disruption of the BBB. Altogether, these results revealed NPS-P88 as a biomarker to evaluate changes in the BBB due to neuroinflammation by MRI in biological models targeting laminin.

Head impact differences in blind football between Rio 2016 and Tokyo 2020 Paralympic Games: video-based observational study

Objective

In Tokyo 2020 Paralympic Games, there were the rule and goal size changes at the blind football competition. This study aimed to compare the scoring and head impact characteristics during blind football competition between the Rio 2016 and Tokyo 2020 Paralympic Games using the official videos.

Design

Video-based observational study.

Participants

In total, 36 blind football (men’s football 5-a-side) game videos were obtained from the official International Paralympic Committee.

Primary and secondary outcome measures

Head impact was defined as the sudden contact of any object with the head. Videos were analysed to assess the number of scores and head impacts along with their corresponding details (ie, round, playing phase, scoring situation, impact situation, occurrence area, impact object, head impact site, fall and foul).

Results

The total number of goals scored at the Tokyo 2020 Paralympic Games was nearly double that at the Rio 2016 Paralympic Games. Regarding head impacts, a total of 2036 cases (Rio 2016, n=1105; Tokyo 2020, n=931) were evaluated. Significant differences were observed in head impact characteristics between the Rio 2016 and Tokyo 2020 Paralympic Games among seven outcomes (round, scoring situation, impact situation, occurrence area, impact object, site of head impact and fall).

Conclusions

Compared with the Rio 2016 Paralympic Games, the Tokyo 2020 Paralympic Games showed an increase in the number of points scored and different head impact characteristics.

Inter- and intra-rater reproducibility of quantitative T1 measurement using semiautomatic region of interest placement in myometrium

Purpose

This study aimed to investigate the inter- and intraobserver reproducibility of quantitative T1 (qT1) measurements using manual and semiautomatic region of interest (ROI) placements. We hypothesized the usefulness of the semiautomatic method, which utilizes a three-dimensional (3D) anatomical relationship between the myometrium and other tissues, for minimizing ROI placement variation, thereby improving qT1 reproducibility compared to the manual approach. The semiautomatic approach, which considered anatomical relationships, was expected to enhance reproducibility by reducing ROI placement variabilities.

Materials and methods

This study recruited 23 healthy female volunteers. Data with variable flip angle (VFA) and inversion recovery were acquired using 3D-spoiled gradient echo and spin echo sequences, respectively. T1 maps were generated with VFA. Manual and semiautomatic ROI placements were independently conducted. Mean qT1 values were calculated from the T1 maps using the corresponding pixel values of the myometrial ROI. Inter- and intraobserver reproducibility of qT1 values was investigated. The inter- and intraobserver reproducibility of qT1 values was evaluated by calculating the coefficient of variation (CoV). Further, reproducibility was evaluated with inter- and intraobserver errors and intraclass correlation coefficients (ICCs). Bland–Altman analysis was utilized to compare the results, estimate bias, and determine the limits of agreement.

Semiautomatic ROI placement demonstrated high reproducibility of qT1 measurements compared with manual methods. Semiautomatic ROI placement may be useful for evaluating uterine qT1 with high reproducibility.

Impact of treatment of hyperkalaemia on quality of life: design of a prospective observational cohort study of long-term management of hyperkalaemia in patients with chronic kidney disease or chronic heart failure in Japan

Introduction

Hyperkalaemia (HK) is a frequent complication in patients with chronic kidney disease (CKD) and/or chronic heart failure (CHF). HK must be managed, both to protect patients from its direct clinical adverse outcomes and to enable treatment with disease-modifying therapies including renin–angiotensin–aldosterone system inhibitors. However, the experiences of patients undergoing treatment of HK are not clearly understood. Optimising treatment decisions and improving long-term patient management requires a better understanding of patients’ quality of life (QOL). Thus, the aims of this research are: (1) to describe treatment patterns and the impact of treatment on a patient’s QOL, (2) to study the relationships between treatment patterns and the impact of treatment on a patient’s QOL and (3) to study the relationships between the control of serum potassium (S-K) and the impact of treatment on a patient’s QOL, in patients with HK.

Methods and analysis

This is a prospective cohort study with 6 months of follow-up in 30–40 outpatient nephrology and cardiology clinics in Japan. The participants will be 350 patients with CKD or CHF who received their first potassium binders (PB) prescription to treat HK within the previous 6 months. Medical records will be used to obtain information on S-K, on treatment of HK with PBs and with diet, and on the patients’ characteristics. To assess the impact of treatment on a patient’s QOL, questionnaires will be used to obtain generic health-related QOL, CKD-specific and CHF-specific QOL, and PB-specific QOL. Multivariable regression models will be used to quantify how treatment patterns and S-K control are related to the impact of treatment on a patient’s QOL.

Ethics and dissemination

Institutional review boards at all participating facilities review the study protocol. Patient consent will be obtained. The results will be published in international journals.

Prácticas de enfermería en la reeducación profesional de los soldados en la Casa Pía de Lisboa durante la Primera Guerra Mundial

Resumen: En el período de la Primera República en Portugal, durante la Primera Guerra Mundial, se implantó la reeducación profesional, basada en la experiencia pedagógica de la Casa Pia de Lisboa, y fue particularmente importante en la recuperación de soldados mutilados. Objetivo: Interpretar y analizar las prácticas de las enfermeras en la reeducación profesional de los soldados portugueses en el Instituto Santa Izabel de Reeducación de Mutilados de Guerra de la Casa Pia de Lisboa durante la Primera Guerra Mundial. Metodología: Utilización del método histórico para sintetizar el relato histórico. Resultados: Se encuentran importantes referencias en los Anuarios de la Casa Pia de Lisboa a las funciones desempeñadas por las enfermeras, a saber: "la propaganda que las enfermeras hacían a los heridos sobre las ventajas de la reeducación"; el "tratamiento de masoterapia"; "los tratamientos de masaje" y también al "importantísimo papel" de las señoras (enfermeras) en la "pequeña enfermería" dirigida por el Dr. V. Pontes, donde había un pequeño servicio de "masoterapia", un taller de "prótesis provisionales" y un laboratorio de examen de aptitudes. Conclusión: Esto confirma que las enfermeras desempeñaron un papel importante en los servicios de "reeducación preparatoria" del Instituto de Reeducación de Mutilados Santa Izabel, de la Casa Pia de Lisboa, interviniendo de forma diferenciada y eficaz, mereciendo un buen reconocimiento.

Burden, determinants, consequences and care of multimorbidity in rural and urbanising Telangana, India: protocol for a mixed-methods study within the APCAPS cohort

Introduction

The epidemiological and demographic transitions are leading to a rising burden of multimorbidity (co-occurrence of two or more chronic conditions) worldwide. Evidence on the burden, determinants, consequences and care of multimorbidity in rural and urbanising India is limited, partly due to a lack of longitudinal and objectively measured data on chronic health conditions. We will conduct a mixed-methods study nested in the prospective Andhra Pradesh Children and Parents’ Study (APCAPS) cohort to develop a data resource for understanding the epidemiology of multimorbidity in rural and urbanising India and developing interventions to improve the prevention and care of multimorbidity.

Methods and analysis

We aim to recruit 2100 APCAPS cohort members aged 45+ who have clinical and lifestyle data collected during a previous cohort follow-up (2010–2012). We will screen for locally prevalent non-communicable, infectious and mental health conditions, alongside cognitive impairments, disabilities and frailty, using a combination of self-reported clinical diagnosis, symptom-based questionnaires, physical examinations and biochemical assays. We will conduct in-depth interviews with people with varying multimorbidity clusters, their informal carers and local healthcare providers. Deidentified data will be made available to external researchers.

Ethics and dissemination

The study has received approval from the ethics committees of the National Institute of Nutrition and Indian Institute of Public Health Hyderabad, India and the London School of Hygiene and Tropical Medicine, UK. Meta-data and data collection instruments will be published on the APCAPS website alongside details of existing APCAPS data and the data access process (www.lshtm.ac.uk/research/centres-projects-groups/apcaps).

Effects of tactical dimension and situational variables in throw-ins on the offensive performance in football

The aim of the study was to describe the usual practices in the execution of throw-ins by La Liga teams during the 2021–2022 season, identify tactical indicators related to the outcome of plays that start with a throw-in, calculate their predictive power, and finally analyse the influence of situational variables on the effectiveness of these plays. A total of 2,658 throw-ins, during 80 matches were analysed. Two UEFA PRO coaches designed an ad hoc observation instrument “Thrinfoot” and two observers coded the data after a training process. Intravenous immunoglobulin treatment in childhood encephalitis (IgNiTE): a randomised controlled trial

Objective

To investigate whether intravenous immunoglobulin (IVIG) improves neurological outcomes in children with encephalitis when administered early in the illness.

Design

Phase 3b multicentre, double-blind, randomised placebo-controlled trial.

Setting

Twenty-one hospitals in the UK.

Participants

Children aged 6 months to 16 years with a diagnosis of acute or subacute encephalitis, with a planned sample size of 308.

Intervention

Two doses (1 g/kg/dose) of either IVIG or matching placebo given 24–36 hours apart, in addition to standard treatment.

Main outcome measure

The primary outcome was a ‘good recovery’ at 12 months after randomisation, defined as a score of≤2 on the Paediatric Glasgow Outcome Score Extended.

Secondary outcome measures

The secondary outcomes were clinical, neurological, neuroimaging and neuropsychological results, identification of the proportion of children with immune-mediated encephalitis, and IVIG safety data.

Results

18 participants were recruited from 12 hospitals and randomised to receive either IVIG (n=10) or placebo (n=8) between 23 December 2015 and 26 September 2017. The study was terminated early following withdrawal of funding due to slower than anticipated recruitment, and therefore did not reach the predetermined sample size required to achieve the primary study objective; thus, the results are descriptive. At 12 months after randomisation, 9 of the 18 participants (IVIG n=5/10 (50%), placebo n=4/8 (50%)) made a good recovery and 5 participants (IVIG n=3/10 (30%), placebo n=2/8 (25%)) made a poor recovery. Three participants (IVIG n=1/10 (10%), placebo n=2/8 (25%)) had a new diagnosis of epilepsy during the study period. Two participants were found to have specific autoantibodies associated with autoimmune encephalitis. No serious adverse events were reported in participants receiving IVIG.

Conclusions

The IgNiTE (ImmunoglobuliN in the Treatment of Encephalitis) study findings support existing evidence of poor neurological outcomes in children with encephalitis. However, the study was halted prematurely and was therefore underpowered to evaluate the effect of early IVIG treatment compared with placebo in childhood encephalitis.

The global prevalence of overweight and obesity among nurses: A systematic review and meta‐analyses

Abstract

Background

Several studies have reported the prevalence of overweight and obesity in various countries but the global prevalence of nurses with overweight and obesity remains unclear. A consolidation of figures globally can help stakeholders worldwide improve workforce development and healthcare service delivery.

Objective

To investigate the global prevalence of overweight and obesity among nurses.

Design

Systematic review with meta-analysis.

Setting

29 different countries across the WHO-classified geographical region.

Participants

Nurses.

Methods

Eight electronic databases were searched for articles published from inception to January 2023. Two independent reviewers performed the article screening, methodological appraisal and data extraction. Methodological appraisal was conducted using Newcastle-Ottawa Scale (NOS). Inter-rater agreement was measured using Cohen's Kappa. Meta-analyses were conducted to pool the effect sizes on overweight, obesity and waist circumference using random effects model and adjusted using generalised linear mixed models and Hartung–Knapp method. Logit transformation was employed to stabilise the prevalence variance. Subgroup analyses were performed based on methodological quality and geographical regions. Heterogeneity was assessed using the I 2 statistic.

Results

Among 10,587 studies, 83 studies representing 158,775 nurses across 29 countries were included. Based on BMI, the global prevalence of overweight and obesity were 31.2% (n = 55, 95% CI: 29%–33.5%; p < .01) and 16.3% (n = 76, 95% CI: 13.7%–19.3%, p < .01), respectively. Subgroup analyses indicated that the highest prevalence of overweight was in Eastern Mediterranean (n = 9, 37.2%, 95% CI: 33.1%–41.4%) and that of obesity was in South-East Asia (n = 5, 26.4%, 95% CI: 5.3%–69.9%). NOS classification, NOS scores, sample size and the year of data collected were not significant moderators.

Conclusions

This review indicated the global prevalence of overweight and obesity among nurses along with the differences between regions. Healthcare organisations and policymakers should appreciate this increased risk and improve working conditions and environments for nurses to better maintain their metabolic health.

High prevalence of overweight and obesity among nurses worldwide.

Prevalencia del catéter venoso periférico en un hospital terciario: elección en función del tratamiento

Objetivo: Determinar la prevalencia del catéter venoso periférico y conocer si su elección es adecuada en función del tratamiento. Metodología: Estudio observacional transversal realizado en la Unidad de Medicina Interna de un hospital de tercer nivel. Análisis descriptivo e inferencial mediante explotación de datos de forma pseudoanonimizada y retrospectiva. Resultados: El 96,6% de pacientes fueron portadores de catéteres venosos periféricos con una media de días de catéter de 3, 62. Los motivos de retirada más frecuentes fueron la extravasación (51,4%), el fin de tratamiento (16,2%) y la flebitis (12,8%). El tratamiento endovenoso fue superior a 6 días en un 40,4% de los pacientes y considerado vesicante y/o irritante en un 56,4%. Conclusiones: Existe un sobre uso del catéter venoso periférico, resultando primordial el diseño e implementación de estrategias con enfoque multidisciplinar que favorezcan una adecuada elección y manejo de los accesos vasculares.

Siguiendo la línea de trabajo sobre la Cultura Organizacional Hospitalaria, en los meses de agosto y septiembre, con motivo de una rotación por el hospital de día de Neurología, he podido realizar un relato biográfico de una persona afectada por Esclerosis Múltiple (EM). Destaco de ella, por un lado, las dificultades que tuvo esta persona para conseguir un diagnóstico y un tratamiento de su enfermedad. En segundo lugar resaltar el papel crucial que demanda el enfermo en su tratamiento y las aportaciones que realiza para una mejor organización del servicio que se ofrece por parte del hospital

Implicaciones del dolor crónico en el bienestar del paciente y diferentes intervenciones para abordarlo

Objetivo principal: Identificar las necesidades que presentan los pacientes con dolor crónico, cómo repercute en su vida y determinar terapias que contribuyen a mejorar el control del dolor. Metodología: Presentación de un caso, mujer de 81años con úlcera venosa asociada a dolor crónico que afecta a su bienestar. Se realizó una revisión bibliográfica en las bases de datos Pubmed, Cinhal, Cuiden y Dialnet llegando a la inclusión de 11 artículos para su análisis. Resultados principales: Los pacientes con dolor crónico ven alteradas sus necesidades físicas, psicológicas y sociales. Se identifican factores que influyen positiva y negativamente en la experiencia con la enfermedad. Se comprueba la existencia de diversas terapias no farmacológicas eficaces en el control de los síntomas. Conclusión principal: El dolor crónico es una enfermedad que afecta de forma holística a la persona y que podría ser abordado con terapias complementarias a la farmacológica.

Cuidados de enfermería ante la adversidad

Nos movemos en un mundo complejo que aúna los mayores avances que el conocimiento científico-tecnológico ha aportado a la humanidad, con las peores situaciones de inseguridad a las que ha sido sometido el ser humano, que adquieren dimensiones planetarias como consecuencia de la globalización. Si las enfermeras sabemos que el compromiso con la ciudadanía se acentúa en los momentos de adversidad, lo cierto es que acontecimientos como desastres naturales, accidentes imprevistos de grandes dimensiones, actos terroristas, conflictos armados y en general situaciones que ponen a prueba la supervivencia de los individuos y las colectividades, nos hacen tomar conciencia de la verdadera extensión a la que somos llamadas en nuestra vocación de servicio [fragmento de texto].
