Exploration of Latina/Hispanic womens experiences living with non-alcoholic fatty liver disease: a qualitative study with patients in Houston

Objectives

A deeper understanding of the lived experiences of Hispanic patients with non-alcoholic fatty liver disease (NAFLD) can help guide the development of behavioural programmes that facilitate NAFLD management. This paper explores Hispanic women’s experiences living with NAFLD.

Design, setting, participants

We collected brief sociodemographic questionnaires and conducted in-depth interviews with 12 low-income (all had household income ≤USD$55 000 per year) Hispanic women with NAFLD from the Houston area. Transcripts were audio-recorded and transcribed. We developed a coding scheme and used thematic analysis to identify emergent themes, supported by Atlas.ti.

Results

Participants identified physicians as their main information source on NAFLD but also consulted the internet, family, friends and peers. Many were still left wanting more information. Participants identified family history, sedentary lifestyles, poor diet and comorbid conditions as causes for their NAFLD. Participants also reported emotional distress after diagnosis. Participants experienced both successes and challenges in making lifestyle changes in nutrition and physical activity. Some participants received desired social support in managing NAFLD, although there were conflicting feelings about spousal support.

Conclusion

Multifaceted programming that improves patient–provider communication, conveys accurate information and enhances social support is needed to support Hispanic women in managing NAFLD.

Association between home meal preparers and salt intake in haemodialysis patients: a cross-sectional study

Objectives

This study aimed to examine the association between home meal preparer and salt intake among haemodialysis patients, including daily dietary status. We hypothesised that salt intake is higher among individuals who rely on meal preparation from others than those who prepare meals by themselves.

Design

Cross-sectional study.

Two medical facilities in Fukushima Prefecture, Japan.

Outcome measure

Salt intake amount was calculated from the results of predialysis and postdialysis blood draws, using Watson’s formula based on predialysis weight, predialysis serum sodium level, postdialysis weight and serum sodium level at the end of dialysis.

Results

Salt intake was significantly higher in participants who relied on meal preparation from others (‘relying on others’) than those who prepared meals by themselves (‘self-prepared’) (B=1.359; 95% CI: 0.495 to 2.222). No statistical difference was found between individuals who ate out or ate takeout (‘outsourcing’) and those who prepared their own meals (‘self-prepared’). These results were robust after adjustment for confounding factors.

Conclusions

The present study revealed an association between self-preparation of meals at home and reduced salt intake among dialysis patients. Our findings suggest that whoever is the home meal preparer is possibly a social determinant of salt intake. To improve the prognosis of haemodialysis patients, actively reaching out to the family and assessing their social environment, such as identifying the home meal preparer and, if the patient relies on others for meal preparation, conducting nutritional/dietary guidance for that person, are effective in enhancing salt reduction.

Bridging the gap: A new species of arboreal <i>Abronia</i> (Squamata: Anguidae) from the Northern Highlands of Chiapas, Mexico

The mountain forests of Middle America are renowned for their endemic biodiversity, and arboreal alligator lizards (genus Abronia) are high-profile vertebrates endemic to this region. In this work, we describe a new species of arboreal Abronia that is known only from the type locality in the Northern Highlands of Chiapas, Mexico. The new species is diagnosed from all other members of the genus Abronia by the following combination of characters: lack of protuberant or spine-like supra-auricular scales, lack of protuberant or casque-like posterolateral head scales, dorsum of head pale yellow with distinct dark markings, 35–39 transverse dorsal scale rows, lateralmost row of ventral scales enlarged relative to adjacent medial row, and dorsum brown with darker crossbands that are sometimes reduced to rows of spots. We provisionally include the new species in the subgenus Lissabronia based on genomic and morphological evidence, but our results also suggest a close relationship to the subgenus Abaculabronia. The new species is geographically separated from the nearest Lissabronia and Abaculabronia species by the lowland Central Depression of Chiapas. Ongoing habitat loss and other factors imperil the new species, leading us to propose its listing under multiple threatened species frameworks. Because the Northern Highlands have poor coverage of protected areas, we briefly comment on the potential of this new species for stimulating conservation in the region.

Impact of COVID-19 on diabetes care: mixed methods study in an Indigenous area of Guatemala

Introduction

SARS-CoV-2 has impacted globally the care of chronic diseases. However, direct evidence from certain vulnerable communities, such as Indigenous communities in Latin America, is missing. We use observational data from a health district that primarily serves people of Maya K’iche’ ethnicity to examine the care of type 2 diabetes in Guatemala during the pandemic.

Methods

We used a parallel convergent mixed methods design. Quantitative data (n=142 individuals with diabetes) included glycated haemoglobin (HbA1c), blood pressure, body mass index and questionnaires on diabetes knowledge, self-care and diabetes distress. Quantitative data was collected at two points, at baseline and after COVID restrictions were lifted. For quantitative outcomes, we constructed multilevel mixed effects models with multiple imputation for missing data. Qualitative data included interviews with providers, supervisors and individuals living with diabetes (n=20). We conducted thematic framework analysis using an inductive approach.

Results

Quantitative data was collected between June 2019 and February 2021, with a median of 487 days between data collection points. HbA1c worsened +0.54% (95% CI, 0.14 to 0.94) and knowledge about diabetes decreased –3.54 points (95% CI, –4.56 to –2.51). Qualitatively, the most important impact of the pandemic was interruption of the regular timing of home visits and peer group meetings which were the standard of care.

Conclusions

The deterioration of diabetes care was primarily attributed to the loss of regular contact with healthcare workers. The results emphasize the vulnerability of rural and Indigenous populations in Latin America to the suspension of chronic disease care.

A mapping review of methicillin-resistant <i>Staphylococcus aureus</i> proportions, genetic diversity, and antimicrobial resistance patterns in Cameroon

Background

The emergence of methicillin-resistant Staphylococcus aureus (MRSA) has increased and poses a significant threat to human and animal health in Cameroon and the world at large. MRSA strains have infiltrated various settings, including hospitals, communities, and livestock, contributing to increased morbidity, treatment costs, and mortality. This evidence synthesis aims to understand MRSA prevalence, resistance patterns, and genetic characterization in Cameroon.

Methods

The methodology was consistent with the PRISMA 2020 guidelines. Studies of any design containing scientific data on MRSA prevalence, genetic diversity, and antimicrobial resistance patterns in Cameroon were eligible for inclusion, with no restrictions on language or publication date. The search involved a comprehensive search strategy in several databases including Medline, Embase, Global Health, Web of Science, African Index Medicus, and African Journal Online. The risk of bias in the included studies was assessed using the Hoy et al tool, and the results were synthesized and presented in narrative synthesis and/or tables and graphs.

Results

The systematic review analyzed 24 studies, mostly conducted after 2010, in various settings in Cameroon. The studies, characterized by moderate to low bias, revealed a wide prevalence of MRSA ranging from 1.9% to 46.8%, with considerable variation based on demographic and environmental factors. Animal (0.2%), food (3.2% to 15.4%), and environmental samples (0.0% to 34.6%) also showed a varied prevalence of MRSA. The genetic diversity of MRSA was heterogeneous, with different virulence gene profiles and clonal lineages identified in various populations and sample types. Antimicrobial resistance rates showed great variability in the different regions of Cameroon, with notable antibiotic resistance recorded for the beta-lactam, fluoroquinolone, glycopeptide, lincosamide, and macrolide families.

Conclusion

This study highlights the significant variability in MRSA prevalence, genetic diversity, and antimicrobial resistance patterns in Cameroon, and emphasizes the pressing need for comprehensive antimicrobial stewardship strategies in the country.

Influence of scanbody design and intraoral scanner on the trueness of complete arch implant digital impressions: An <i>in vitro</i> study

This study aimed to compare the accuracy of full-arch digital implant impressions using seven different scanbodies and four intraoral scanners. A 3D-printed maxillary model with six implants and their respective multi-unit abutments was used for this study. Seven scanbodies (SB1, SB2, SB3, SB4, SB5, SB6, and SB7) and four intraoral scanners (Primescan®, Omnican®, Trios 3®, and Trios 4®) were assessed. Each combination group was scanned ten times and a dental lab scanner (D2000, 3Shape) was used as a reference. All scans were exported as STL files, imported into Convince software (3Shape) for alignment, and later into Blender software, where their 3D positions were analyzed using a Python script. The 3D deviation, angular deviation, and linear distance between implants #3 and #14 were also measured. Prevalence and molecular heterogeneity of glucose-6-phosphate dehydrogenase (G6PD) deficiency in the Senoi Malaysian Orang Asli population

Glucose-6-phosphate dehydrogenase (G6PD) deficiency is an X-linked genetic disorder characterized by reduced G6PD enzyme levels in the blood. This condition is common in populations exposed to malaria; an acute febrile disease caused by Plasmodium parasites. G6PD-deficient individuals may suffer from acute hemolysis following the prescription of Primaquine, an antimalarial treatment. The population at risk for such a condition includes the Senoi group of Orang Asli, a remote indigenous community in Malaysia. This study aimed to elucidate the G6PD molecular heterogeneity in this subethnic group which is important for malaria elimination. A total of 662 blood samples (369 males and 293 females) from the Senoi subethnic group were screened for G6PD deficiency using a quantitative G6PD assay, OSMMR2000-D kit with Hb normalization. After excluding the family members, the overall prevalence of G6PD deficiency in the studied population was 15.2% (95% CI: 11–19%; 56 of 369), with males (30 of 172; 17.4%) outnumbering females (26 of 197; 13.2%). The adjusted male median (AMM), defined as 100% G6PD activity, was 11.8 IU/gHb. A total of 36 participants (9.6%; 26 male and 10 female) were deficient (G)(39.1%; n = 34), G6PD Viangchan (c.871G>A)(25.3%; n = 22), G6PD Union (c.1360C>T)(21.8%; n = 19), c.1311C>T(20.7%; n = 18), G6PD Kaiping (c.1388G>A)(8.0%; n = 7), and G6PD Coimbra (c.592C>T)(2.3%; n = 2). Our analysis revealed 27 hemizygote males, 18 heterozygote females, 7 homozygote females, and 2 compound heterozygote females. This study confirms the high prevalence of G6PD deficiency among the Senoi Malaysian Orang Asli, with a significant degree of molecular heterogeneity. More emphasis should be placed on screening for G6PD status and proper and safe use of Primaquine in the elimination of malaria among this indigenous population.

A comparative analysis of converters of tabular data into image for the classification of Arboviruses using Convolutional Neural Networks

Tabular data is commonly used in business and literature and can be analyzed using tree-based Machine Learning (ML) algorithms to extract meaningful information. Deep Learning (DL) excels in data such as image, sound, and text, but it is less frequently utilized with tabular data. However, it is possible to use tools to convert tabular data into images for use with Convolutional Neural Networks (CNNs) which are powerful DL models for image classification. The goal of this work is to compare the performance of converters for tabular data into images, select the best one, optimize a CNN using random search, and compare it with an optimized ML algorithm, the XGBoost. Results show that even a basic CNN, with only 1 convolutional layer, can reach comparable metrics to the XGBoost, which was trained on the original tabular data and optimized with grid search and feature selection. However, further optimization of the CNN with random search did not significantly improve its performance.

Firearm injuries in Missouri

Firearm deaths continue to be a major public health problem, but the number of non-fatal firearm injuries and the characteristics of patients and injuries is not well known. The American College of Surgeons Committee on Trauma, with support from the National Collaborative on Gun Violence Research, leveraged an existing data system to capture lethal and non-lethal injuries, including patients treated and discharged from the emergency department and collect additional data on firearm injuries that present to trauma centers. In 2020, Missouri had the 4th highest firearm mortality rate in the country at 23.75/100,000 population compared to 13.58/100,000 for the US overall. We examined the characteristics of patients from Missouri with firearm injuries in this cross-sectional study. Of the overall 17,395 patients, 1,336 (7.7%) were treated at one of the 11 participating trauma centers in Missouri during the 12-month study period. Patients were mostly male and much more likely to be Black and uninsured than residents in the state as a whole. Nearly three-fourths of the injuries were due to assaults, and overall 7.7% died. Few patients received post-discharge services.

Evaluating the impact of COVID-19 pandemic-related home confinement on the refractive error of school-aged children in Germany: a cross-sectional study based on data from 414 eye care professional centres

Objective

This study aimed at evaluating refractive changes in German school-aged children before and after the COVID-19 pandemic.

Design

Cross-sectional study.

Setting

414 eye care professional centres from Germany.

Participants

Refractive data from 59 926 German children aged 6–15 years were examined over a 7-year period (2015–2021).

Primary and secondary outcome measures

Spherical equivalent refraction was assessed as a function of year, age and gender. The refractive values concerning 2020 and 2021 were compared with those assigned to prior years (2015–2019).

Results

Disruption of normal lifestyle due to pandemic-related home confinement appears to lead to a myopic refractive shift in children aged 6–11 years in Germany. The greater effect observed at younger ages seems to emphasise the importance of refractive development in this age group.

Effects of 36 hours of sleep deprivation on military-related tasks: Can ammonium inhalants maintain performance?

Introduction

A lack of sleep can pose a risk during military operations due to the associated decreases in physical and cognitive performance. However, fast-acting ergogenic aids, such as ammonia inhalants (AI), may temporarily mitigate those adverse effects of total sleep deprivation (TSD). Therefore, the present study aimed to investigate the acute effect of AI on cognitive and physical performance throughout 36 hours of TSD in military personnel.

Methods

Eighteen male military cadets (24.1 ± 3.0 y; 79.3 ± 8.3 kg) performed 5 identical testing sessions during 36 hours of TSD (after 0 [0], 12 [–12], 24 [–24], and 36 [–36] hours of TSD), and after 8 [+8] hours of recovery sleep. During each testing session, the following assessments were conducted: Epworth sleepiness scale (ESS), simple reaction time (SRT), shooting accuracy (SA), rifle disassembling and reassembling (DAS), and countermovement jump height (JH). Heart rate (HR) was continuously monitored during the SA task, and a rating of perceived exertion (RPE) was obtained during the JH task. At each time point, tests were performed twice, either with AI or without AI as control (CON), in a counterbalanced order.

Although there were detrimental effects of TSD, the usage of AI did not reduce those adverse effects. However, regardless of TSD, AI did result in a short-term increase in HR, improved SRT without affecting the number of errors, and improved JH while concurrently decreasing the RPE. No changes, yet, were observed in SA and DAS. These results suggest that AI could potentially be useful in some military scenarios, regardless of sleep deprivation.

Contributing to self‐care of a person with chronic obstructive pulmonary disease: A qualitative study of the experiences of family caregivers

Abstract

Aim

To explore family caregivers' experiences of contributing to self-care of patients with chronic obstructive pulmonary disease (COPD).

Design

A qualitative description study.

Methods

Individual semi-structured interviews were conducted face-to face, by telephone or video calls in a purposive sample of 17 family caregivers of patients with COPD recruited in Italy, and analysed through content analysis. The consolidated criteria for reporting qualitative studies (COREQ) checklist was used for study reporting.

Results

Ten subcategories were derived from 106 codes grouped into three main categories: family caregiver contributions to maintaining disease stable and ensuring a normal life for patients; family caregiver contributions to disease monitoring; and family caregiver contributions to coping with disease exacerbations. Family caregivers provided practical and emotional support, and their contribution was essential to improve treatment adherence, to enable the patient to continue living a normal life, and to have access to the healthcare services. Family caregivers were constantly vigilant and monitored patients daily to detect worsening conditions, and they managed exacerbations especially when patients were unable to do it due to their critical conditions.

Conclusion

This study broadens knowledge of family caregivers' contributions to patients' self-care in COPD, describing the different ways family members provide daily care to patients and the many responsibilities they take on.

Impact

Family caregivers perform a variety of behaviours when supporting patients with COPD in self-care, especially when patients are more dependent and the disease more severe. Nurses should acknowledge the various contributions provided by family caregivers and develop educational interventions aiming to support them in patient care and improve patient outcomes.

Precancerous cervical lesion screening acceptance among women in Eastern Ethiopia

Objective

This study assessed perceived barriers, precancerous cervical lesion screening acceptance, and associated factors among women in Eastern Ethiopia.

Setting

This study was conducted in Hiwot Fana Specialized Hospital and Jugal Hospital.

Study design

This study is a facility-based cross-sectional study.

Study participants

This study included 1181 women aged 25–49 years. Bivariate and multivariable logistic regression was used to evaluate the relationship between variables and control confounders.

Results

Nearly half of the participants (587 or 49.7%) agreed to be screened for precancerous cervical lesions. Seventy-six per cent of those checked were negative for visual inspection with acetic acid, 18.5% were positive, and 5.7% had cancer-like lesions. In multivariable analysis, fear of discomfort from the screening procedure, having a male screener, and embarrassment were the perceived barriers that were inversely associated with screening acceptance.

Conclusions

The uptake of the screening service in the study area was not satisfactory, indicating that the programme was underutilised in the area.

Validación de contenido “Escala de autoeficacia para el amamantamiento, formato corto” en mujeres puérperas

Objetivo. Determinar la validez de contenido de la Escala de Autoeficacia para el Amamantamiento en mujeres puérperas. Metodología. Diseño metodológico de validación, con cinco etapas: 1. Adaptación sociocultural de la escala; 2. Validación de contenido; 3. Prueba piloto; 4. Factibilidad pragmática mediante Ensayo Clínico Aleatorio; 5. Análisis factorial exploratorio. Resultados. Etapa 1-2: Los jueces modificaron en los 14 ítems, la palabra “poder”, por la palabra "Tengo la confianza". Índice de Validez por Ítem (1.7); Criterio de Validez (12%) y el Índice de Validez de Contenido (8.78). Etapa 3-4: Prueba piloto, α=.85. Se encontraron diferencias entre el grupo control y experimental en el re-test (p< .05). Etapa 5: Las subescalas explican el 39.91% de la varianza. Conclusión principal. Instrumento válido y confiable para medir la autoeficacia para el amamantamiento en mujeres puérperas. La intervención educativa y persuasión verbal mejora significativamente el nivel de autoeficacia mediante el componente educativo.

Conductas del estilo de vida y sarcopenia en personas mayores de Mexicali México: estudio transversal

Objetivo principal: Describir la prevalencia de sarcopenia y analizar la influencia de las conductas asociadas del estilo de vida en la sarcopenia en personas mayores residentes de Mexicali, México. Metodología: Se consideró un diseño descriptivo inferencial. Participaron 105 personas mayores de 60 años o más. El muestreo fue no probabilístico. Se evaluaron los criterios diagnósticos para sarcopenia y las conductas asociadas al estilo de vida. La investigación se realizó acorde a la reglamentación ética. Resultados principales: El 24,8% y el 59% presentaron uno de los tres estadios de la sarcopenia acorde a los puntos de corte para la población mexicana y EWGSOP respectivamente. Se encontró asociación con el IMC (p <,01) y sexo (p <,05). Conclusión principal: Se identificó que la prevalencia es menor cuando se consideran los puntos de corte regionales para México. El IMC y el sexo se asociaron con la sarcopenia.

El cuidado de enfermería en el puerperio inmediato en la práctica de la lactancia materna: experiencia del enfermero

Objetivo principal: comprender la percepción de las enfermeras sobre la experiencia de asistencia ofrecida a la lactancia materna en el puerperio inmediato. Metodología: estudio de abordaje cualitativo, apoyado en etnografía, basado en etno-enfermería. La investigación se realizó en la maternidad de un hospital ubicado en el sur de Minas Gerais, con siete enfermeras que desarrollan prácticas de atención relacionadas con la mujer en el puerperio inmediato. Resultados principales: surgieron ocho categorías temáticas y sus respectivas sub-categorías: Lactancia materna y beneficios; ¡El parto normal ayuda! ¡El parto por cesárea se interpone! Práctica en asistencia a la lactancia materna; Atención prenatal y hospitalaria; Pautas; Lactancia materna en el puerperio inmediato; Dificultades enfrentadas durante la lactancia materna y percepción de la importancia del rol de la enfermería. Conclusión principal: las enfermeras, a pesar de tener práctica y conocimientos, se dan cuenta de que los cuidados de enfermería aún son deficientes, señalando muchas barreras que dificultan la práctica de la lactancia materna.

Intervenciones de enfermería a Recién Nacido Extremadamente Prematuro con Síndrome de Distrés Respiratorio

Introducción: La prematuridad y el bajo peso al nacer son factores predictivos asociados a la morbilidad y la mortalidad neonatal y, por lo tanto, dan lugar a complicaciones como el Síndrome de Dificultad Respiratoria (SDR), que es la principal complicación de los recién naci-dos que ingresan en la Unidad de Cuidados Intensivos Neonatales (UCIN). Objetivo: implementar cuidados integrales de enfermería especializados e individualizados al recién nacido extremadamente prematuro basados en recomendaciones de evidencia científica e intervenciones NIC. Presentamos la valoración integral de una persona recién nacida de 23 SDG con un peso de 500 g. más SDR remitidos desde un hospital comunitario básico a un hospital de tercer nivel de atención, donde se aplica el proceso de enfermería. Métodos: Se seleccionó un paciente a conveniencia de la UCIN, aplicando la metodología del proceso de enfermería, la evaluación integral a tra-vés de los 13 dominios de la NANDA, los datos se obtuvieron del expediente clínico, la entrevista con la madre y el examen físico del recién nacido Se identifican las principales respuestas humanas para analizar datos objetivos y subjetivos, se hacen juicios clínicos para implementar planes de atención bajo recomendaciones de guías de práctica clínica e intervenciones de NIC. Resultados: Se implementa-ron cinco planes de atención con diagnóstico priorizado de enfermería, cuatro reales y uno en riesgo, se identificaron los dominios más alterados: nutrición, eliminación/intercambio, crecimiento/desarrollo y seguridad/protección. Conclusiones: se realizaron intervenciones independientes e interdependientes observando mejoría en los dominios alterados, sin embargo, el recién nacido se mantuvo en una incubadora y permaneció en la UCIN para seguir los planes terapéuticos especializados, Esto implica que la UCIN debe contar con personal multidisciplinario altamente capacitado con conocimientos actualizados en atención neonatal.
