Comparing nurses attending a specialised mental health programme with and without substance use disorder: a retrospective, observational study in Spain

Objectives

To analyse the differences between nurses with and without substance use disorders (SUDs) admitted to a specialised mental health programme.

Design

Retrospective, observational study.

Setting

Specialised mental health treatment programme for nurses in Catalonia, Spain.

Participants

1091 nurses admitted to the programme from 2000 to 2021.

Interventions

None.

Primary and secondary outcomes

Sociodemographic, occupational and clinical variables were analysed. Diagnoses followed Diagnostic and Statistical Manual of Mental Disorders, 4th edition, Text Revision criteria.

Results

Most nurses admitted to the programme were women (88%, n=960) and came voluntarily (92.1%, n=1005). The mean age at admission was 45 (SD=10.4) years. The most common diagnoses were adjustment disorders (36.6%, n=399), unipolar mood disorders (25.8%, n=282), anxiety disorders (16.4%, n=179) and SUDs (13.8%, n=151). Only 19.2% (n=209) of the sample were hospitalised during their first treatment episode. After multivariate analysis, suffering from a SUD was significantly associated with being a man (OR=4.12; 95% CI 2.49 to 6.82), coming after a directed referral (OR=4.55; 95% CI 2.5 to 7.69), being on sick leave at admission (OR=2.21; 95% CI 1.42 to 3.45) and needing hospitalisation at the beginning of their treatment (OR=12.5; 95% CI 8.3 to 20).

Conclusions

Nurses with SUDs have greater resistance to voluntarily asking for help from specialised mental health treatment programmes and have greater clinical severity compared with those without addictions. SUDs are also more frequent among men. More actions are needed to help prevent and promote earlier help-seeking behaviours among nurses with this type of mental disorder.

INvestigational Study Into Transplantation of the Uterus (INSITU): a cross-sectional survey among women with uterine factor infertility in the UK assessing background, motivations and suitability

Importance

The study summarises the selection prescreen criteria currently used in the UK for a uterus transplant and highlights the number of women who are suitable to proceed.

Objectives

To assess the demographics, motivations, reasons and suitability among women with absolute uterine factor infertility (AUFI) to undergo uterine transplantation (UTx).

Design

A cross-sectional survey.

Setting

An electronic questionnaire was sent via email to women with AUFI who had previously been referred to the UTx research team or approached the Womb Transplant UK Charity. The questions assessed suitability to undergo UTx based on demographic information, perceptions to adoption and surrogacy and reasons why UTx was preferable. Responses were assessed against the study selection criteria.

Results

210 women completed the questionnaire. The most common aetiology of AUFI in our cohort was Mayer-Rokitansky-Küster-Hauser (68%; n=143) whereas 29% (n=62) had previously undergone hysterectomy. 63% (n=132) of the cohort had previously considered adoption, 5% (n=11) had attempted it and 2 (1%) had successfully adopted. The most common reason cited to undergo UTx over adoption was to experience gestation (n=63; 53%), while 37% (n=44) wanted a biologically related child. 76% (n=160) of participants had previously considered surrogacy, 22 (10%) had attempted it and 2 (1%) had successfully become mothers using a surrogate. The most common reason to undergo UTx over surrogacy was to experience gestation (n=77; 54%). 15% (n=21) were concerned about the legal implications, 14% (n=20) identified the financial cost as a barrier and 8% (n=12) could not consider it due to religious reasons. On adhering to the selection criteria, 65 (31%) women were suitable to proceed with the trial.

Conclusion

The study demonstrates that implementing commonly used selection criteria for a UTx led to an attrition rate of more than two-thirds of women who requested to initially undergo the process. As more studies present outcomes following UTx, critical assessment of the selection criteria currently used is warranted to ensure potential recipients are not being unnecessarily excluded.

La luz y el ruido nocturno en pacientes hospitalizados, ¿afectan al sueño nocturno?

Objetivo. Describir la prevalencia de pacientes con alteración de sueño en presencia de luz y ruidos nocturnos durante su internación en el 6º piso del Hospital Universitario Austral. Material y Métodos. Estudio transversal en una población de pacientes obstétricos y clínicos internados, elegidos mediante muestreo consecutivo. Para la recolección de datos se realizó una encuesta anónima por medio de una herramienta digital. Se incluyeron las variables sociodemográficas y ocho preguntas relacionadas a la satisfacción del descanso y la repercusión de la luz y el ruido de las últimas dos noches. Los datos fueron volcados en un Excel para su posterior análisis. Resultados. La prevalencia de pacientes con alteración del descanso en presencia de luz y de ruido fue del 9% (23). La población analizada fue de 246 pacientes, con una edad promedio de 34 años (DE ± 12); de las cuales 78% (191) fueron mujeres. En cuanto al tipo de paciente, 74% (180) fueron obstétricos y 17% (40) clínicos. Según la ubicación de las habitaciones, 34% (82) se encontraban ubicados cerca del office de enfermería y 66% (160) se encontraban lejos. La alteración del descanso de los pacientes no varía según el tipo de pacientes (obstétricos o clínicos) ni según su cercanía al office de enfermería. Discusión. La prevalencia de la alteración del sueño es muy baja. El instrumento midió la satisfacción con un instrumento corto, rápido, dinámico. Es la única herramienta encontrada en la literatura que responda a la satisfacción y percepción del sueño en relación con el ruido y la luz.

 

ABSTRACT

Objective. Describe the prevalence of inpatients with sleep alterations due to nocturnal lights and noises during their stay in the 6th floor of the Hospital Universitario Austral. Methodology. A cross-sectional study was conducted on a group of inpatients, either from the obstetric or the general care section, who were selected throughout consecutive sampling. Data recollection was done by an anonymous survey using a digital tool. The survey included socio demographic variables and eight questions about rest satisfaction and the impact of light and noise over rest within the last two nights. This information was transferred to an Excel for its further analysis. Results. The prevalence of patients with rest alteration in presence of light and noise was 9% (23). The study was carried out on a group of 246 patients, with an average age of 34 years old (± 12); 78% of which (191) were women. As regards the kind of patient, 74% (180) were obstetrical patients and 17% (40) were derived from the general care section. As to room location, 34% (82) were near the nurse working station and 66% (160) were far away from it. Sleep alteration in patients is not affected by the type of patient (obstetrical or general) or by their proximity to the nurse working station. Discussion. The prevalence of sleep alteration is very low. Satisfaction was measured with a short, quick and dynamic instrument. This is the only document found in literature which answers to rest satisfaction and perception in relation to light and noise.

 



¿Existen cuestionarios de enfermería para predecir el deterioro neurológico en la Esclerosis Lateral Amiotrófica?

Objetivo principal: identificar la existencia de escalas de medición validadas que nos ayuden a predecir la existencia de deterioro neurológico del origen respiratorio y digestivo. Metodología: se realizó una revisión bibliográfica en diciembre de 2019 en las principales bases de datos. Resultados principales: se incluyeron artículos que describen instrumentos para la detección precoz del deterioro neurológico y la calidad de vida en pacientes con Esclerosis Lateral Amiotrófica. Se incluyeron 15 artículos que nos brindaron información sobre los ocho cuestionarios más importantes disponibles y validados para evaluar la progresión de la esclerosis lateral amiotrófica (EuroQoL, SF-12, SPB, ALSFR, ALSFRS-R, NPS, DMN, ALSAQ40). Conclusión principal: a pesar del trabajo constante para mejorar la calidad de vida y la dignidad de los pacientes y sus familias, actualmente no existe un cuestionario validado que prediga la existencia de deterioro neurológico respiratorio y digestivo.
