Impact of heat on mental health emergency visits: a time series study from all public emergency centres, in Curitiba, Brazil

Objectives

Quantify the risk of mental health (MH)-related emergency department visits (EDVs) due to heat, in the city of Curitiba, Brazil.

Design

Daily time series analysis, using quasi-Poisson combined with distributed lag non-linear model on EDV for MH disorders, from 2017 to 2021.

Setting

All nine emergency centres from the public health system, in Curitiba.

Participants

101 452 EDVs for MH disorders and suicide attempts over 5 years, from patients residing inside the territory of Curitiba.

Main outcome measure

Relative risk of EDV (RREDV) due to extreme mean temperature (24.5°C, 99th percentile) relative to the median (18.02°C), controlling for long-term trends, air pollution and humidity, and measuring effects delayed up to 10 days.

Results

Extreme heat was associated with higher single-lag EDV risk of RREDV 1.03(95% CI 1.01 to 1.05—single-lag 2), and cumulatively of RREDV 1.15 (95% CI 1.05 to 1.26—lag-cumulative 0–6). Strong risk was observed for patients with suicide attempts (RREDV 1.85, 95% CI 1.08 to 3.16) and neurotic disorders (RREDV 1.18, 95% CI 1.06 to 1.31). As to demographic subgroups, females (RREDV 1.20, 95% CI 1.08 to 1.34) and patients aged 18–64 (RREDV 1.18, 95% CI 1.07 to 1.30) were significantly endangered. Extreme heat resulted in lower risks of EDV for patients with organic disorders (RREDV 0.60, 95% CI 0.40 to 0.89), personality disorders (RREDV 0.48, 95% CI 0.26 to 0.91) and MH in general in the elderly ≥65 (RREDV 0.77, 95% CI 0.60 to 0.98). We found no significant RREDV among males and patients aged 0–17.

Conclusion

The risk of MH-related EDV due to heat is elevated for the entire study population, but very differentiated by subgroups. This opens avenue for adaptation policies in healthcare: such as monitoring populations at risk and establishing an early warning systems to prevent exacerbation of MH episodes and to reduce suicide attempts. Further studies are welcome, why the reported risk differences occur and what, if any, role healthcare seeking barriers might play.

Effect of PRISMA 2009 on reporting quality in systematic reviews and meta-analyses in high-impact dental medicine journals between 1993–2018

Introduction

The PRISMA guidelines were published in 2009 to address inadequate reporting of key methodological details in systematic reviews and meta-analyses (SRs/MAs). This study sought to assess the impact of PRISMA on the quality of reporting in the full text of dental medicine journals.

Methods

This study assessed the impact of PRISMA (2009) on thirteen methodological details in SRs/MAs published in the highest-impact dental medicine journals between 1993–2009 (n = 211) and 2012–2018 (n = 618). The study further examined the rate of described use of PRISMA in the abstract or full text of included studies published post- PRISMA and the impact of described use of PRISMA on level of reporting. This study also examined potential effects of inclusion of PRISMA in Instructions for Authors, along with study team characteristics.

Results

The number of items reported in SRs/MAs increased following the publication of PRISMA (pre-PRISMA: M = 7.83, SD = 3.267; post-PRISMA: M = 10.55, SD = 1.4). Post-PRISMA, authors rarely mention PRISMA in abstracts (8.9%) and describe the use of PRISMA in the full text in 59.87% of SRs/MAs. The described use of PRISMA within the full text indicates that its intent (guidance for reporting) is not well understood, with over a third of SRs/MAs (35.6%) describing PRISMA as guiding the conduct of the review. However, any described use of PRISMA was associated with improved reporting. Among author team characteristics examined, only author team size had a positive relationship with improved reporting.

Conclusion

Following the 2009 publication of PRISMA, the level of reporting of key methodological details improved for systematic reviews/meta-analyses published in the highest-impact dental medicine journals. The positive relationship between reference to PRISMA in the full text and level of reporting provides further evidence of the impact of PRISMA on improving transparent reporting in dental medicine SRs/MAs.

Protocol for the PATHOME study: a cohort study on urban societal development and the ecology of enteric disease transmission among infants, domestic animals and the environment

Introduction

Global morbidity from enteric infections and diarrhoea remains high in children in low-income and middle-income countries, despite significant investment over recent decades in health systems and water and sanitation infrastructure. Other types of societal development may be required to reduce disease burden. Ecological research on the influence of household and neighbourhood societal development on pathogen transmission dynamics between humans, animals and the environment could identify more effective strategies for preventing enteric infections.

Methods and analysis

The ‘enteric pathome’—that is, the communities of viral, bacterial and parasitic pathogens transmitted from human and animal faeces through the environment is taxonomically complex in high burden settings. This integrated cohort-exposure assessment study leverages natural socioeconomic spectrums of development to study how pathome complexity is influenced by household and neighbourhood infrastructure and hygiene conditions. We are enrolling under 12-month-old children in low-income and middle-income neighbourhoods of two Kenyan cities (Nairobi and Kisumu) into a ‘short-cohort’ study involving repeat testing of child faeces for enteric pathogens. A mid-study exposure assessment documenting infrastructural, behavioural, spatial, climate, environmental and zoonotic factors characterises pathogen exposure pathways in household and neighbourhood settings. These data will be used to inform and validate statistical and agent-based models (ABM) that identify individual or combined intervention strategies for reducing multipathogen transmission between humans, animals and environment in urban Kenya.

Ethics and dissemination

The protocols for human subjects’ research were approved by Institutional Review Boards at the University of Iowa (ID-202004606) and AMREF Health Africa (ID-ESRC P887/2020), and a national permit was obtained from the Kenya National Commission for Science Technology and Innovation (ID# P/21/8441). The study was registered on Clinicaltrials.gov (Identifier: NCT05322655) and is in pre-results stage. Protocols for research on animals were approved by the University of Iowa Animal Care and Use Committee (ID 0042302).
