Frontline nurse leaders' competences in evidence‐based healthcare: A scoping review

Abstract

Aim

To identify evidence on frontline nurse leaders' competences in evidence-based healthcare (EBHC) and the instruments measuring these competences.

Design

A scoping review.

Data Sources

The search was conducted in June 2021 and complemented in June 2022. The CINAHL, ProQuest, Medline (Ovid), Scopus, Web of Science databases and MedNar along with the Finnish database Medic were searched.

Review Method

The scoping review was conducted in accordance with the Joanna Briggs institute methodology for scoping reviews. Titles, abstracts and full-text versions were screened independently by two reviewers according to the inclusion criteria. Deductive-inductive content analysis was used to synthesize data.

Results

A total of 3211 articles published between 1997 and 2022 were screened, which resulted in the inclusion of 16 articles. Although frontline nurse leaders had a positive attitude towards EBHC, they had a lack of implementing EBHC competence into practice. Part of the instruments were used in the studies, and only one focused especially on leaders. None of instruments systematically covered all segments of EBHC.

Conclusion

There is a limited understanding of frontline nurse leaders' competence in EBHC. It is important to understand the importance of EBHC in healthcare and invest in the development of its competence at all levels of leaders. Frontline nurse leaders' support is essential for direct care nurses to use EBHC to ensure the quality of care and benefits to patients. Leaders must enhance their own EBHC competence to become role models for direct care nurses. It is also essential to develop valid and reliable instruments to measure leaders' competence covering all EBHC segments. The results can be utilized in the assessment and development of frontline nurse leaders' EBHC competence by planning and producing education and other competence development methods.

Uncertainty in the association between socio-demographic characteristics and mental health

Questionnaires are among the most basic and widespread tools to assess the mental health of a population in epidemiological and public health studies. Their most obvious advantage (firsthand self-report) is also the source of their main problems: the raw data requires interpretation, and are a snapshot of the specific sample’s status at a given time. Efforts to deal with both issues created a bi-dimensional space defined by two orthogonal axes, in which most of the quantitative mental health research can be located. Methods aimed to assure that mental health diagnoses are solidly grounded on existing raw data are part of the individual validity axis. Tools allowing the generalization of the results across the entire population compose the collective validity axis. This paper raises a different question. Since one goal of mental health assessments is to obtain results that can be generalized to some extent, an important question is how robust is a questionnaire result when applied to a different population or to the same population at a different time. In this case, there is deep uncertainty, without any a priori probabilistic information. The main claim of this paper is that this task requires the development of a new robustness to deep uncertainty axis, defining a three-dimensional research space. We demonstrate the analysis of deep uncertainty using the concept of robustness in info-gap decision theory. Based on data from questionnaires collected before and during the Covid-19 pandemic, we first locate a mental health assessment in the space defined by the individual validity axis and the collective validity axis. Then we develop a model of info-gap robustness to uncertainty in mental health assessment, showing how the robustness to deep uncertainty axis interacts with the other two axes, highlighting the contributions and the limitations of this approach. The ability to measure robustness to deep uncertainty in the mental health realm is important particularly in troubled and changing times. In this paper, we provide the basic methodological building blocks of the suggested approach using the outbreak of Covid-19 as a recent example.

Alcohol policies in India: A scoping review

Globally, alcohol consumption causes significant societal harm and is a leading risk factor for death and disability in adults. In India, 3.7% of all deaths and 3.1% disability adjusted life years (DALYs) can be attributed to alcohol. In the context of rapid economic development and emphasized by the COVID-19 pandemic, India’s lack of a consolidated and comprehensive alcohol policy has posed significant challenges to addressing this harm. In this context, the aim of our review was to undertake a comprehensive mapping of the State and national policy environment surrounding alcohol and its use in India, based on an analysis of policy documents. We did this though a scoping review of academic and grey literature, which helped to iteratively identify the websites of 15 international organizations, 21 Indian non-governmental organizations, and eight Indian Federal governmental organizations as well as State/Union Territory government sites, to search for relevant policy documents. We identified 19 Federal policy documents and 36 State level policy documents within which we have identified the specific policy measures which address the 10 categories of the World Health Organization’s Global Action Plan to Reduce the Harmful Use of Alcohol. We found that there are major gaps in regulation of marketing and price controls, with much of this controlled by the States. In addition, regulation of availability of alcohol varies widely throughout the country, which is also a policy area controlled locally by States. Through the clear elucidation of the current policy environment surrounding alcohol in India, policy makers, researchers and advocates can create a clearer roadmap for future reform.

Six-month post-intensive care outcomes during high and low bed occupancy due to the COVID-19 pandemic: A multicenter prospective cohort study

Introduction

The COVID-19 pandemic can be seen as a natural experiment to test how bed occupancy affects post-intensive care unit (ICU) patient’s functional outcomes. To compare by bed occupancy the frequency of mental, physical, and cognitive impairments in patients admitted to ICU during the COVID-19 pandemic.

Methods

Prospective cohort of adults mechanically ventilated >48 hours in 19 ICUs from seven Chilean public and private hospitals. Ninety percent of nationwide beds occupied was the cut-off for low versus high bed occupancy. At ICU discharge, 3- and 6-month follow-up, we assessed disability using the World Health Organization Disability Assessment Schedule 2.0. Quality of life, mental, physical, and cognitive outcomes were also evaluated following the core outcome set for acute respiratory failure.

Results

We enrolled 252 participants, 103 (41%) during low and 149 (59%) during high bed occupancy. Patients treated during high occupancy were younger (P50 [P25-P75]: 55 [44–63] vs 61 [51–71]; p Conclusions

There were no differences in post-ICU outcomes between high and low bed occupancy. Most patients (>90%) had at least one mental, physical or cognitive impairment at ICU discharge, which remained high at 6-month follow-up (57%).

Multimodal personalised executive function intervention (E-Fit) for school-aged children with complex congenital heart disease: protocol for a randomised controlled feasibility study

Introduction

Children with congenital heart disease (CHD) are at risk for executive functions (EF) impairments. To date, interventions have limited effects on EF in children and adolescents with complex CHD. Therefore, we developed a new multimodal and personalised EF intervention (E-Fit). This study aims to test the feasibility of this intervention called ‘E-Fit’ for children with complex CHD and EF impairments.

Methods and analysis

This is a single-centre, single-blinded, randomised controlled feasibility study exploring the E-Fit intervention. We aim to enrol 40 children with CHD aged 10–12 years who underwent infant cardiopulmonary bypass surgery and show clinically relevant EF impairments (T-score ≥60 on any Behaviour Rating Inventory for Executive Function questionnaire summary scale). The multimodal intervention was developed with focus groups and the Delphi method involving children and adolescents with CHD, their parents and teachers, and health professionals. The intervention is composed of three elements: computer-based EF training using CogniFit Inc 2022, performed three times a week at home; weekly EF remote strategy coaching and analogue games. The content of the computer and strategy training is personalised to the child’s EF difficulties. The control group follows their daily routines as before and completes a diary about their everyday activities four times a week. Participants will be randomised in a 1:1 ratio. Feasibility is measured by the participants’ and providers’ ratings of the participants’ adherence and exposure to the intervention, recruitment rates and the evaluation of the intended effects of the programme.

Ethics and dissemination

Local ethics committee approval was obtained for the study (BASEC-Nr: 2021-02413). Parents provide written informed consent. Key outputs from the trial will be disseminated through presentations at conferences, peer-reviewed publications and directly to participating families. Furthermore, these results will inform the decision whether to proceed to a randomised controlled trial to investigate effectiveness.

Lifecourse investigation of the cumulative impact of adversity on cognitive function in old age and the mediating role of mental health: longitudinal birth cohort study

Objective

To investigate the accumulation of adversities (duration of exposure to any, economic, psychosocial) across the lifecourse (birth to 63 years) on cognitive function in older age, and the mediating role of mental health.

Design

National birth cohort study.

Setting

Great Britain.

Participants

5362 singleton births within marriage in England, Wales and Scotland born within 1 week of March 1946, of which 2131 completed at least 1 cognitive assessment.

Main outcome measures

Cognitive assessments included the Addenbrooke’s Cognitive Examination-III, as a measure of cognitive state, processing speed (timed-letter search task), and verbal memory (word learning task) at 69 years. Scores were standardised to the analytical sample. Mental health at 60–64 years was assessed using the 28-item General Health Questionnaire, with scores standardised to the analytical sample.

Results

After adjusting for sex, increased duration of exposure to any adversity was associated with decreased performance on cognitive state (β=–0.39; 95% CI –0.59 to –0.20) and verbal memory (β=–0.45; 95% CI –0.63 to –0.27) at 69 years, although these effects were attenuated after adjusting for further covariates (childhood cognition and emotional problems, educational attainment). Analyses by type of adversity revealed stronger associations from economic adversity to verbal memory (β=–0.54; 95% CI –0.70 to –0.39), with a small effect remaining even after adjusting for all covariates (β=–0.18; 95% CI –0.32 to –0.03), and weaker associations from psychosocial adversity. Causal mediation analyses found that mental health mediated all associations between duration of exposure to adversity (any, economic, psychosocial) and cognitive function, with around 15% of the total effect of economic adversity on verbal memory attributable to mental health.

Conclusions

Improving mental health among older adults has the potential to reduce cognitive impairments, as well as mitigate against some of the effect of lifecourse accumulation of adversity on cognitive performance in older age.

Behavioural outcomes of children born with intrauterine growth restriction: protocol for a systematic review and meta-analysis

Introduction

Intrauterine growth restriction (IUGR) is a pregnancy condition, which is associated with poor perinatal outcomes and long-term neurodevelopmental impairment. Several studies also investigated the impact of IUGR on child behaviour (eg, internalising and externalising behaviour, social competencies). However, so far, no systematic review or meta-analysis has been conducted that summarises these effects while considering relevant third variables such as type of IUGR diagnosis and control group, or concurrent cognitive abilities. The objective of this study is to summarise the current evidence regarding the relationship between IUGR and behavioural outcomes from early childhood to young adulthood. Additionally, to explore how third variables such as type of control group, or cognitive abilities, relate to this association.

Methods

Search strategy: The following electronic databases will be searched—Web of Science, Medline Ovid, PsycInfo, Cochrane Library, Scopus and Embase. Inclusion criteria: observational (eg, cohort studies and case–control studies) and intervention studies (if standard care is used and norm values are reported for the control group) will be included if they quantitatively compare children with and without IUGR from the age of 2 to 18 years. The main outcomes are internalising and externalising behaviour, and social competencies.

Ethics and dissemination

No ethics approval was necessary for this protocol. Dissemination of findings will be done by publishing the results in peer-reviewed journals. The results of this systematic review will provide guidance for practice and counselling for clinicians and therapists facing patients affected by IUGR and their families.

Home-based model is an important cost-effective alternative to centre-based cardiac rehabilitation

    Validación de contenido “Escala de autoeficacia para el amamantamiento, formato corto” en mujeres puérperas

    Objetivo. Determinar la validez de contenido de la Escala de Autoeficacia para el Amamantamiento en mujeres puérperas. Metodología. Diseño metodológico de validación, con cinco etapas: 1. Adaptación sociocultural de la escala; 2. Validación de contenido; 3. Prueba piloto; 4. Factibilidad pragmática mediante Ensayo Clínico Aleatorio; 5. Análisis factorial exploratorio. Resultados. Etapa 1-2: Los jueces modificaron en los 14 ítems, la palabra “poder”, por la palabra "Tengo la confianza". Índice de Validez por Ítem (1.7); Criterio de Validez (12%) y el Índice de Validez de Contenido (8.78). Etapa 3-4: Prueba piloto, α=.85. Se encontraron diferencias entre el grupo control y experimental en el re-test (p< .05). Etapa 5: Las subescalas explican el 39.91% de la varianza. Conclusión principal. Instrumento válido y confiable para medir la autoeficacia para el amamantamiento en mujeres puérperas. La intervención educativa y persuasión verbal mejora significativamente el nivel de autoeficacia mediante el componente educativo.

    Tiempo de respuesta al timbre; una oportunidad para mejorar el flujo de trabajo

    Introducción. Actualmente el timbre conecta a los pacientes con la expectativa de una asistencia inmediata cuando perciben una necesidad, ya sea para asistencia de rutina o tengan un cambio agudo en su condición. El tiempo de atención al timbre impacta en la calidad de atención, satisfacción del paciente, y el flujo de trabajo. El objetivo fue describir el tiempo promedio de la respuesta al timbre en la Unidad Coronaria. Metodología. Estudio transversal del tiempo de respuesta al timbre estratificado por turno. Se recogieron datos sociodemográficos de los pacientes, los motivos de la llamada al timbre, y la satisfacción de los pacientes de forma anónima y digital. Resultados. Se analizaron 200 timbres, con un tiempo promedio de 5’42”. El promedio de edad de los pacientes que accionaron el timbre fue de 56 años, de los cuales 51% eran mujeres. Los principales motivos de llamada fueron la movilización (24.9%) e higiene/eliminación (19.2%), y los pacientes se mostraron satisfechos siempre o casi siempre (81%) con el tiempo de atención al timbre. No se halló una asociación entre los días de internación y la cantidad de llamadas al timbre. Discusión. A pesar de que el tiempo es superior a los “minutos dorados”, los pacientes presentan un elevado grado de satisfacción. La variabilidad de los tiempos en los distintos turnos y los principales motivos de llamada por turno muestran un enorme desafío en la gestión de enfermería para anticipar y priorizar las necesidades que el paciente transmite a través del timbre.

    Abstract

    Introduction. Currently, the call light connects patients with the expectation of immediate assistance when they perceive a need, whether it is for routine assistance or a sudden change in their condition. The response time to the call bell impacts the quality of care, patient satisfaction, and workflow. The objective of this study was to describe the average response time to the call bell in the Coronary Unit. Methodology. A cross-sectional study of response time to the call bell stratified by shift, over 3 days, with a minimum of 7 days between each day. Sociodemographic data of patients, reasons for the call bell, and patient satisfaction were collected anonymously and digitally. Results. 200 call bells were analyzed, with an average response time of 5'42". The average age was 56 years, with 51% being female. The main reasons for calling were mobilization (24.9%) and hygiene/elimination (19.2%). Most patients were satisfied with the response time to the call bell (81% always or almost always). There was no association found between length of stay and the number of call bells. Discussion. Despite the response time being longer than the "golden minutes," patients show a high degree of satisfaction. The variability of response times in different shifts and the main reasons for calling by shift show a huge challenge in nursing management to anticipate and prioritize the needs that the patient expresses through the call bell.

    Family engagement in paediatric acute care settings: A realist review

    Aim

    To create a programme theory of family engagement in paediatric acute care to explicate the relationships between contexts and mechanisms of family engagement that align with family, direct care providers and healthcare organization outcomes.

    Design

    Realist review and synthesis.

    Data Sources

    PubMed, CINAHL, PsycINFO and Web of Science searches for the 2.5-year period (July 2019–December 2021) following our 2021 scoping review.

    Review Methods

    Following methods described by Pawson and Rycroft-Malone, we defined the scope of the review, searched for and appraised the evidence, extracted and synthesized study findings and developed a supporting narrative of our results.

    Results

    Of 316 initial citations, 101 were included in our synthesis of the final programme theory. Contexts included family and direct care provider individualism, and the organizational care philosophy and environment. Mechanisms were family presence, family enactment of a role in the child's care, direct care providers facilitating a family role in the child's care, unit/organizational promotion of a family role, relationship building and mutually beneficial partnerships. Outcomes were largely family-focussed, with a paucity of organizational outcomes studied. We identified four context–mechanism–outcome configurations.

    Conclusion

    This realist review uncovered underlying contexts and mechanisms between patients, direct care providers and organizations in the family engagement process and key components of a mutually beneficial partnership. Given that successful family engagement requires direct care provider and organizational support, future research should expand beyond family outcomes to include direct care providers, particularly nurses and healthcare organization outcomes.

    Impact

    The final programme theory of family engagement in paediatric acute care provides a roadmap for clinicians to develop complex interventions to engage families and evaluate their impact. The components of our final programme theory reflect family engagement concepts that have been evolving for decades.

    Patient or Public Contribution

    The team conducting this review included members from the practice setting (JT & KG). In the future, as we and others use this model in practice, we will seek input for refinement from clinicians, patients and caregivers.

    Impacto de las lesiones por presión en un paciente post Covid-19

    Objetivo principal: Explorar el impacto de las lesiones por presión (LPP) en las personas después de un ingreso en la UCI por Covid-19. Metodología: Estudio de caso bajo enfoque cualitativo. Se realizó una entrevista semiestructurada para explorar el fenómeno “padecer LPP en el contexto de la enfermedad por Covid-19” y un análisis temático mediante la teoría fundamentada. Resultados principales: El análisis resultó en las siguientes categorías: signos y síntomas, funcionamiento físico, bienestar psicológico, funcionamiento social, vivencia del proceso, ámbito de los cuidados, espiritualidad, tratamientos, elementos de cura experta, comunicación de la información, satisfacción de cuidados, proceso de alta, integración y déficits de conocimientos. Conclusión principal: El estudio destaca el gran impacto que las lesiones por presión causan en todas las dimensiones calidad de vida, además de en las esferas espiritual, económica y la necesidad de información.

    Supporting Parkinson's disease medication safety for nurses in the acute care setting through an educational intervention study

    Abstract

    Introduction

    Patient medication safety in the acute care setting is a foundational action provided by nurses and healthcare providers for safe patient care. Hospitalization of patients with Parkinson's disease (PD) can be dangerous due to the unique and variable medication regimen required. Patients with PD often have their medication administered inappropriately in the acute care setting (e.g., holding a PD medication in preparation for surgery, not administering the medication on the patient's home schedule, and delaying administration). The research question posed in this study was the following: does a PD medication educational intervention in the clinical setting enhance knowledge, comfort, and competence of practicing nurses in the care of patients with PD regarding their medication safety?

    Design

    A mixed methods study design was used for this 5-month, two-part study with a sample of practicing RNs at three different hospitals. Part one of the study assessed nurses' initial knowledge of PD and PD medication safety and included an educational intervention. Part two of the study occurred 3 months later and evaluated if knowledge from the educational intervention was retained.

    Methods

    The study was conducted in two parts and included a pre-test, educational intervention, post-test, and follow-up test 3 months later. The educational intervention consisted of a 15-minute video of two PD advanced practice nurses being interviewed regarding the general care of a patient with PD. The pre-test, post-test, and follow-up test were identical and consisted of six questions regarding knowledge, comfort, and self-perceived competency. Participants were additionally asked three open-ended questions at follow-up to gain insight on the effectiveness of the educational intervention.

    Results

    A total sample of 252 RNs participated in this study. Statistically significant improvements in knowledge, comfort, and self-perceived competency were observed in the post-test scores compared to pre-test scores. These statistically significant improvements were retained after 3 months, despite a 42.9% decrease in the number of responders (n = 252 vs. n = 144). Additionally, compared to the post-test, there were no statistically significant declines in knowledge, comfort, or competency in the follow-up test. Qualitative findings indicated that the training regarding PD medications was retained and found to be valuable, even if it was seldom applied in practice.

    Conclusion

    A review of the literature and this study both support the need for increased education for practicing nurses as it relates to PD and PD medication safety. Healthcare systems, organizations, and associations that support continuing education for nurses create a stronger workforce. Education has been found to keep nurses up to date on the latest advances in care and treatment while also providing exposure to other areas of nursing beyond their clinical settings.

    Clinical Relevance

    Promoting better patient outcomes through safe medication administration is a hallmark of nursing care excellence. This study found that supporting the use of an educational intervention of PD medication safety for nurses improved RN levels of knowledge, comfort, and competency up to 3 months later. As the population of those with PD increases, healthcare systems, and nurses must now, more than ever, be poised to care for these individuals. This is a critical point in PD patient care since persons with PD are hospitalized 1.5 times more than their peers without PD.

    Conductas del estilo de vida y sarcopenia en personas mayores de Mexicali México: estudio transversal

    Objetivo principal: Describir la prevalencia de sarcopenia y analizar la influencia de las conductas asociadas del estilo de vida en la sarcopenia en personas mayores residentes de Mexicali, México. Metodología: Se consideró un diseño descriptivo inferencial. Participaron 105 personas mayores de 60 años o más. El muestreo fue no probabilístico. Se evaluaron los criterios diagnósticos para sarcopenia y las conductas asociadas al estilo de vida. La investigación se realizó acorde a la reglamentación ética. Resultados principales: El 24,8% y el 59% presentaron uno de los tres estadios de la sarcopenia acorde a los puntos de corte para la población mexicana y EWGSOP respectivamente. Se encontró asociación con el IMC (p <,01) y sexo (p <,05). Conclusión principal: Se identificó que la prevalencia es menor cuando se consideran los puntos de corte regionales para México. El IMC y el sexo se asociaron con la sarcopenia.

    Preparación preoperatoria: la percepción de la persona que se somete a una cirugía programada

    Objetivo principal: Conocer la percepción de la persona sometida a una cirugía general programada sobre su preparación preoperatoria. Método: Estudio cualitativo y exploratorio. Se realizaron diez entrevistas semiestructuradas con personas que se habían sometido a una cirugía general programada en los últimos seis meses en un hospital universitario de la región central de Portugal. Para analizar los datos se utilizó la técnica del análisis de contenido. Resultados principales: Surgieron cinco unidades temáticas, percepción de la experiencia quirúrgica, sentimientos/emociones percibidas, necesidades de información, expectativas sobre los cuidados de enfermería preoperatorios y perspectivas sobre la existencia de una consulta de enfermería, que se subdividieron en varias categorías y subcategorías. Conclusión principal: Algunas personas siguen viendo la experiencia quirúrgica como algo traumático y consideran el entorno quirúrgico como un generador de estrés. Este estudio refuerza la relevancia del personal de enfermería en la preparación de las personas en situaciones perioperatorias y puede proporcionar importantes contribuciones a la estructuración de una consulta de enfermería.

    Assessing the fulfillment of nurses' full potential in diverse geriatric settings—moving towards change

    Abstract

    Background

    In response to the aging population, the Department for Professional Development in the Nursing Division at the Israeli Ministry of Health has invested significant resources in training registered nurses for post-basic certification in the field of geriatrics. However, little is known about how the nurses implement the scope of practice in the field and how they maximize their full potential following the training.

    Aims and objectives

    To assess whether nurses' full potential is maximized in geriatric acute and long-term geriatric care, and community settings in Israel.

    Design

    A quantitative, cross-sectional study.

    Methods

    The study population is a convenience sample of registered nurses who completed post-basic certification in geriatric care (n = 287). The participants completed a self-administered questionnaire addressing socio-demographic and occupational characteristics, implementation of the scope of practice, and a professional satisfaction questionnaire.

    Baseline demographic variables were summarized using means and counts, with frequency counts and percentages used for the scope of practice. A one-way ANOVA was utilized to examine the differences between the workplace and the scope of practice. Chi-square was used to examine practice types and the workplace.

    Results

    Nurses in all surveyed geriatric settings reported average to high overall performance in independent decision-making activities and delegated duties. The majority (66%) reported very high satisfaction with the completion of the post basic certification education However overall performance differed significantly between the different geriatric settings.

    Conclusion

    Most of the nurses with a post-basic geriatric certification are satisfied with their scope of practice, feel valued by colleagues and managers, and are able to practice independently within their professional scope. This can have a positive impact on the care of the older adult population.

    Clinical Relevance

    Administrators should ensure that nurses with a post-basic geriatric certification are able to maximize their scope of practice. Specifically, we recommend nurse managers to leverage the results of this study and advocate for nurses to fulfill their full potential in their scope of practice.

    Factores relacionados con conducta suicida en personas hospitalizadas en una unidad psiquiátrica

    Objetivo. Analizar factores relacionados con la conducta suicida en pacientes con enfermedad mental para orientar la prevención y cuidados de salud mental especializados. Material y métodos. Estudio descriptivo transversal de una muestra de 44 pacientes hospitalizados por conducta suicida en psiquiatría. Durante 12 meses se registraron variables sociodemográficas y clínicas, se aplicaron la escala Columbia de cribado del riesgo suicida, la de Desesperanza de Beck, la de Impulsividad de Plutchik y el Inventario de Razones para Vivir. Se usó estadística descriptiva e inferencial para una significación estadística de p<0,05. Resultados. El 84% (n=37) presentó ideación suicida, el 63,3% (n=28) tentativa y el 2,3% (n=1) suicidio consumado. Realizaron más intentos previos las mujeres (60%/n=15) que los hombres (38%/n=7). La intoxicación medicamentosa fue el método más utilizado por ambos sexos seguido de autolesiones por mujeres (p<0,05) y precipitación por hombres (p<0,05). La depresión, trastorno más prevalente, se asoció a la edad y al medio rural (p<0,05). El trastorno de ansiedad fue el más predictivo para la intención/plan y tentativas suicidas (p<0,05). El 80% (n=16) de los intentos padecía enfermedad médica (p<0,05). Se evidenció la relación inversa entre las creencias de supervivencia/afrontamiento y la conducta suicida. Discusión. La conducta suicida supone más del 20% de los ingresos de una unidad psiquiátrica. No es posible determinar un perfil único de riesgo, se han evidenciado diferencias según el espectro suicida. Todos los factores relacionados deben ser valorados.

     

    ABSTRACT

    Objective. To analyze factors related to suicidal behavior in patients with mental illness to guide prevention and specialized mental health care. Methodology. A descriptive cross-sectional study carried out in a sample of 44 inpatients at a psychiatric unit who attempted suicide. Sociodemographic and clinical variables were registered during 12 months. The scales applied were the Columbia-Suicide Severity Rating Scale (C-SSRS), Beck Hopelessness Scale, Plutchik’s Impulsivity Scale and the Reasons for Living Inventory. A descriptive and inferential statistic has been applied for a statistical significance of p<0,05. Results. 84% (n=37) had suicidal thinking, 63,3% (n=28) attempted suicide and 2,3% (n=1) committed suicide. More women (60%; n=15) than men (38%; n=7) attempted suicide. Medication overdose was the main suicidal approach by both sexes followed by self-harm by women (p<0,05) and deliberate fall by men (p<0,05). Depression, the most prevalent disorder, was associated with age and rural environment (p<0,05). Anxiety had the highest predictive value of suicidal attempt (p<0,05). 80% (n=16) of those who attempted suicide suffered from a medical illness (p<0,05). There is evidence of inverse relationship between the survival and coping beliefs and the suicidal behaviour. Discussion. Suicidal behaviour surpasses 20% of the admissions at a psychiatric unit. It is not possible to determine a specific risk profile as differences according to the suicidal spectrum have been acknowledged. Every possible cause must be evaluated.

    Vocación para el cuidado humano en estudiantes de enfermería en una universidad de México

    Objetivo: Comparar el nivel de vocación en estudiantes de segundo, cuarto, sexto y octavo semestre de la licenciatura en enfermería de una universidad privada en México. Metodología: Diseño comparativo, transversal; la muestra fueron 127 estudiantes de pregrado en enfermería de diferentes semestres. Se utilizó el instrumento “Vocación de servicio para el cuidado humano”, una cédula de datos; se usó el consentimiento informado. El procesamiento de datos se realizó en SPSS, se utilizó estadística descriptiva e inferencial. Resultados: 86% fueron mujeres, el promedio de edad fue 21.1±4, el promedio de felicidad fue 8.7±1.4, 60,6% tiene una buena vocación; existen diferencias entre el nivel de vocación de octavo semestre respecto a los demás (p<.05), existe relación entre la felicidad y vocación (r=.385; p<.01). Conclusión: El nivel de vocación en los estudiantes de Enfermería es bueno, existen diferencias significativas entre el nivel de vocación a medida que aumentan los semestres. La felicidad y la vocación se relacionan y condicionan potencialmente el desempeño profesional.

    Detección precoz de lesiones oculares en una unidad de cuidados intensivos pediátricos: Elaboración y validación de un cuestionario

    Objetivo principal: Elaboración y validación de un cuestionario sobre detección precoz de lesiones oculares en el paciente crítico pediátrico. Metodología: Estudio descriptivo de validación de contenido de un cuestionario. Se seleccionó bibliografía científica, se redactaron los ítems y se validó el contenido mediante un panel de 8 expertos con experiencia en unidades de cuidados intensivos pediátricos. Resultados principales: 53 artículos relacionados con la aparición de daño ocular en unidades de cuidados intensivos fueron seleccionados. Se agruparon los factores de riesgo y se incluyeron aquellos que superaron la valoración de un panel de expertos. Conclusión principal: el contenido de este cuestionario para detectar lesiones oculares en unidades de cuidados intensivos pediátricos es un instrumento repre-sentativo, por lo que puede usarse en estudios posteriores para establecer su fiabilidad, validez, viabilidad y sensibilidad al cambio.

    Perspectiva del profesional de enfermería sobre la gestión sanitaria durante el Estado de Alarma por la COVID-19

    En España, la COVID-19 ha supuesto un incremento en la presión asistencial y un gran desafío para las administraciones sanitarias. Objetivo: El objetivo de este estudio fue estudiar la perspectiva del personal de enfermería sobre la gestión sanitaria en la Comunidad Autónoma de Aragón durante el Estado de Alarma por la COVID-19. Metodología: Estudio observacional descriptivo transversal retros-pectivo llevado a cabo en 428 enfermeras del Servicio Aragonés de Salud a través de un cuestionario elaboración propia, estructurado y heteroadministrado. Resultados: La valoración sobre la actuación del Gobierno de España, el Gobierno de Aragón y el Servicio Aragonés de Salud obtuvieron una puntuación media de 3,60; 4,17 y 3,84 respectivamente. Conclusión: La gestión sanitaria llevada a cabo por las autoridades competentes durante el Estado de Alarma, fue ineficiente desde la perspectiva del personal de enfermería.
