Self‐Care Behaviors in Osteoporosis: A Cross‐Cultural Study Using Multidimensional Analysis of Textual Data

ABSTRACT

Introduction

Osteoporosis is a chronic skeletal condition that significantly affects daily life and may benefit from adequate self-care. The Middle-Range Theory of Self-Care of Chronic Illness conceptualizes self-care as a naturalistic process encompassing maintenance, monitoring, and management, also shaped by contextual and cultural factors. Despite this theoretical framework, limited evidence exists on how people with osteoporosis conceptualize and enact self-care in different cultural contexts. Italy and Spain provide comparable yet distinct settings for exploring cultural influences on self-care practices. This study aims to explore and compare self-care behaviors among Italian and Spanish people with osteoporosis.

Design

A cross-national qualitative study was conducted using an Automatic Analysis of Textual Data approach within the Exploratory Multidimensional Data Analysis framework.

Methods

Forty people with osteoporosis (38 women and 2 men; mean age 68.6 years, SD = 5.74) were recruited, equally distributed between Italy (n = 20) and Spain (n = 20). Data were analyzed using Labbé's intertextual distance and correspondence analysis to examine lexical similarities, differences, and semantic structures of self-care narratives.

Results

Although moderate intra-national lexical similarity was observed within each national group, substantial inter-national differences emerged between Italian and Spanish participants in the discursive construction of self-care. Italian participants described self-care as a medically mediated and system-oriented process, closely linked to professional guidance, diagnostic procedures, and prescribed treatments. In contrast, Spanish participants framed self-care as embodied, routine-based, and emotionally expressive, grounded in daily practices and bodily awareness. These findings indicate that participants relied on distinct cultural and epistemic models when interpreting and organizing self-care experiences.

Conclusion

This study highlights clear cross-cultural differences in how people with osteoporosis narrate and conceptualize self-care. Methodologically, it illustrates the potential of multidimensional analysis to support systematic and transparent comparison of qualitative datasets. The findings are consistent with the Middle-Range Theory of Self-Care and suggest that greater attention to cultural contexts may inform the development of self-care interventions that are more responsive to individuals' lived experiences.

Clinical Relevance

Recognizing how cultural contexts influence how people with osteoporosis understand and practise self-care may help nurses tailor communication and educational support to individuals' everyday experiences, supporting more responsive and culturally sensitive care.

Molecular analysis and computational modeling reveal temporally separable responses triggered by DENV-induced soluble factors in endothelial cells

Dengue virus (DENV) represents a growing global health challenge with billions of people at risk. Severe Dengue (SD), a complication of DENV infection that involves generalized hemorrhage, is driven, at least in part, by endothelial dysfunction. Endothelial dysfunction refers to increased permeability due to inflammation, mechanical injury and/or modification of the genetic program of endothelial cells. Previous work showed that exposure of endothelial cells to conditioned media from DENV-infected cells (CMDV) increased permeability and cellular stiffness, repressed endothelial markers and induced mesenchymal genes. However, the generality, extent, mechanism and ultimate impact of these events in the onset of SD remain elusive. Here, we integrate analysis from in vitro treatment of endothelial cells with media containing UV-inactivated DENV with computational modeling to investigate the key features of CMDV-induced endothelial alterations and their potential impact on endothelial dysfunction. We found that CMDV increased SNA1 and CDH2 expression, while suppressing endothelial genes OCLN and CDH5. Global transcriptomics analysis revealed that CMDV triggered a transient pro-inflammatory response, followed by induction of selected tissue repair genes and matrix remodeling. A non-directed asynchronous network model (NDAM-CMDV) identified IL6 and FN1 as central nodes of DENV-induced endothelial trans-differentiation, providing new molecular insights that predict the evolution of the disease and identify potential therapeutic targets.

Stewarding scarce response capacity: an inductive qualitative interview study of emergency medical dispatchers prioritising ambulance resources

Objective

This study aimed to explore emergency medical dispatchers’ (EMDs’) experiences of prioritising patients and stewarding ambulance resources when system capacity was constrained.

Design

Qualitative interview study using inductive qualitative content analysis.

Setting

Emergency medical communication centres (EMCCs) in Sweden, operated by the national emergency call provider and responsible for receiving 112 calls and dispatching ambulances.

Participants

13 purposively sampled EMDs with at least 1 year of professional experience.

Data analysis

Interviews were analysed inductively using qualitative content analysis (Elo and Kyngäs) through open coding, grouping into subcategories and abstraction into generic categories and one main category.

Results

Dispatchers described prioritisation under scarcity as system work that simultaneously addressed individual patient acuity and population-level readiness. One main category captured this work: stewarding scarce response capacity. Three inter-related generic categories characterised stewardship: (1) prioritising by clinical urgency within geographic and operational constraints; (2) producing availability through anticipation, reassessment and queue governance in a ‘virtual waiting room’; and (3) coordinating response through information infrastructures and interprofessional collaboration. Across categories, dispatchers described redistributing risk across patients and time, managing moral strain when delays could harm patients and using experience, reassessment and teamwork to avoid both under-response to urgent need and over-allocation that would leave areas without coverage.

Conclusions

Dispatch under scarcity is best understood as active stewardship of a safety-critical dispatch queue. Strengthening patient safety therefore requires organisational support for reassessment and escalation during prolonged waits, explicit governance of queue dynamics and geographic coverage trade-offs, safeguards for contextual judgement when automation is used and support for dispatchers exposed to morally difficult scarcity decisions.

La enfermería ante el desafío del cuidado humanizado y el liderazgo profesional: un análisis comparativo de los códigos de ética y deontología de la profesión enfermera

Este artículo presenta un análisis comparativo de ocho códigos de ética y deontología de enfermería, explorando cómo conceptualizan el cuidado humanizado, el liderazgo enfermero, la gestión enfermera y el profesionalcentrismo.

Objetivo: Realizar un análisis comparativo de nueve códigos de ética y deontología de enfermería, tanto españoles como internacionales, para examinar cómo abordan cuatro dimensiones clave de la profesión.

Metodología: Se analizaron códigos de ética y deontología españoles (Consejo General de Enfermería, el borrador del Código Deontológico Nacional de Enfermería, el Codi d'Ètica de les infermeres i infermers de Catalunya, Código de Ética y Deontología de la Enfermería de Navarra, Código Ético y Deontológico de la Enfermería de la Comunitat Valenciana) e internacionales (Consejo Internacional de Enfermería, Federación Europea de Órganos Reguladores de Enfermería y American Nursing Association).

Resultados: El análisis revela que la humanización del cuidado es un pilar universal y central en la práctica de enfermería, con un enfoque consistente en la dignidad, autonomía y bienestar del paciente. Se observa una evolución en el reconocimiento del liderazgo y la gestión enfermera, pasando de funciones implícitas a competencias explícitas y proactivas, especialmente en los códigos españoles más recientes.

Conclusiones: Esta evolución refleja un profesionalismo maduro que busca empoderar a la enfermería para liderar y gestionar recursos eficazmente, siempre con el objetivo de promover un cuidado humanizado y de alta calidad, sin caer en un profesionalcentrismo perjudicial. La enfermera es reafirmada como un agente fundamental en la humanización del cuidado, trascendiendo la mera intervención técnica.

A Comprehensive Assessment of the Environmental Impact of Different Infant Feeding Types: The Observational Study GREEN MOTHER

ABSTRACT

Aim(s)

To observe and compare the environmental impacts of different types of infant feeding, considering the use of formula, infant feeding accessories, potentially increased maternal dietary intake during breastfeeding (BF) and food consumption habits.

Design

An observational cross-sectional multicentre study conducted in the Barcelona Metropolitan Area of the Catalan Institute of Health.

Methods

Data were collected from 419 postpartum women on infant feeding type (formula milk and accessories), maternal dietary intake (24-h register) and food consumption habits from November 2022 to April 2023. The environmental impacts (climate change (CC), water consumption and water scarcity) of the infant feeding types and maternal diet were calculated using the IPCC, ReCiPE and AWARE indicators, respectively. The differences in impacts were calculated by Kruskal–Wallis test.

Results

Significant differences for the three environmental impacts were observed. The CC impact of formula milk and feeding accessories was 0.01 kg CO2eq for exclusive BF, 1.55 kg CO2eq for mixed feeding and 4.98 kg CO2eq for formula feeding. While BF mothers consumed an extra 238 kcal, no significant differences were found related to maternal diet across feeding types.

Conclusion

Exclusive BF was the most sustainable type of infant feeding, considering formula and infant feeding accessories. In our study, the difference between the impacts of BF and non-BF mothers' diet was insignificant.

Satisfacción materna con la atención recibida durante el parto y puerperio y su relación con factores asociados

Objetivo. Evaluar el grado de satisfacción con los cuidados brindados durante el parto y el posparto en el Hospital Álvaro Cunqueiro (HAC) y explorar su posible relación con variables sociodemográficas y obstétricas. Metodología. Se realizó un estudio observacional descriptivo transversal. La población incluyó mujeres con partos entre enero y octubre de 2024 en el HAC. El muestreo fue no probabilístico y de tipo consecutivo. Para la recolección de datos, se utilizó un cuestionario ad hoc que recopiló información sobre variables sociodemográficas y obstétricas, complementado con el instrumento “Care in Obstetrics: Measure for Testing Satisfaction” (COMFORTS) en su versión validada en español. Resultados. La muestra estuvo compuesta por 319 mujeres. La mediana de satisfacción global obtenida con el cuestionario COMFORTS fue de 171 (RIC: 155–186), lo que indica un alto nivel de satisfacción general. Sin embargo, las áreas de cuidados en el posparto [Me: 46 (RIC: 40–53)] y al recién nacido [Me: 40 (RIC: 30–46)] fueron identificadas como susceptibles de mejora. Se encontró una relación estadísticamente significativa entre la paridad y la percepción de la calidad de los cuidados neonatales, así como entre el modo de finalización del parto y el grado de satisfacción materna con la experiencia del parto. Discusión. Aunque la satisfacción general con los cuidados obstétricos en el HAC es alta, existen áreas de mejora en el posparto y en la atención al recién nacido. Además, variables como la paridad y el modo de finalización del parto influyen en la percepción materna de los cuidados.

Sampling was non-probabilistic and consecutive. For data collection, an ad hocquestionnaire was used to obtain information on sociodemographic and obstetric variables, complemented with the instrument Care in Obstetrics: Measure for Testing Satisfaction (COMFORTS) in its validated Spanish version. Results.The sample consisted of 319 women. The median overall satisfaction with the COMFORTS questionnaire was 171 (IQR: 155–186), indicating a high level of overall satisfaction. However, the areas of postpartum [Me: 46 (IQR: 40–53)] and newborn care [Me: 40 (IQR: 30–46)] were identified as areas for improvement. A statistically significant relationship was found between parity and perceived quality of neonatal care, as well as between the mode of delivery and maternal satisfaction with the birth experience. Discussion: Although overall satisfaction with obstetric care at the HAC is high, there are areas for improvement in postpartum and newborn care. In addition, variables such as parity and mode of delivery influence maternal perceptions of care.

The Experience of Self‐Care in People With Osteoporosis: A Qualitative Descriptive Study

ABSTRACT

Introduction

Osteoporosis requires long-term self-care engagement, yet little is known about how individuals experience and manage self-care in everyday life. Understanding these experiences is essential to inform tailored nursing interventions. The objective of the study was to explore and describe the experience of self-care maintenance, monitoring, and management in people with osteoporosis.

Design

A qualitative descriptive study.

Methods

We conducted semi-structured interviews. Data were analyzed using Mayring's qualitative content analysis with a deductive approach based on Riegel's theory of self-care. We reported data in accordance with the Consolidated Criteria for Reporting Qualitative Studies (COREQ) checklist.

Results

Participants (1 Male, 19 Females; Aged 55–80) Identified Four Themes of self-care: maintenance (e.g., Medication Adherence, Physical Activity), monitoring (e.g., Symptom Recognition, Test Interpretation), management (e.g., Lifestyle Reflections, Prevention), and general self-care. Key factors included motivation, trust in healthcare professionals, and integration of health behaviors into daily life. Barriers were low self-efficacy, poor symptom recognition, and inconsistent adherence.

Conclusion

Self-care in osteoporosis is a multidimensional and dynamic process influenced by individual beliefs, contextual factors, and support from healthcare professionals. Recognizing the variability in patients' self-care behaviors is essential to develop personalized education and support. Strengthening general health behaviors may enhance disease-specific self-care. This understanding can guide healthcare professionals in designing more effective, tailored care strategies.

Detección precoz de lesiones oculares en una unidad de cuidados intensivos pediátricos: Elaboración y validación de un cuestionario

Objetivo principal: Elaboración y validación de un cuestionario sobre detección precoz de lesiones oculares en el paciente crítico pediátrico. Metodología: Estudio descriptivo de validación de contenido de un cuestionario. Se seleccionó bibliografía científica, se redactaron los ítems y se validó el contenido mediante un panel de 8 expertos con experiencia en unidades de cuidados intensivos pediátricos. Resultados principales: 53 artículos relacionados con la aparición de daño ocular en unidades de cuidados intensivos fueron seleccionados. Se agruparon los factores de riesgo y se incluyeron aquellos que superaron la valoración de un panel de expertos. Conclusión principal: el contenido de este cuestionario para detectar lesiones oculares en unidades de cuidados intensivos pediátricos es un instrumento repre-sentativo, por lo que puede usarse en estudios posteriores para establecer su fiabilidad, validez, viabilidad y sensibilidad al cambio.

Sala azul: espacio de aprendizaje vivencial

O objetivo foi analisar as contribuições do processo de aprendizagem vivencial para o desenvolvimento do cuidado sensível no estudante de enfermagem. Pesquisa descritiva, qualitativa, desenvolvida a partir dos registros do formulário de avaliação preenchido por estudantes do terceiro, oitavo e novo período. Foram obtidas amostras não intencionais, dos anos de 2013, 2014 e 2015, totalizando 298 participantes. O campo de estudo foi a Sala Azul, situado na Faculdade de Enfermagem da Universidade Estadual do Rio de Janeiro. Buscou-se respeitar os procedimentos ético-legais que envolvem pesquisas com seres humanos. Emergiram três categorias: Espaço de reflexão para o cuidado de si e do outro; Espaço de aprendizagem da empatia multidimensional e Espaço que promove redução do estresse através do relaxamento. Os resultados sugerem que o processo de aprendizagem vivencial possibilita o desenvolvimento do autoconhecimento, o cuidado de si e do outro, as habilidades empáticas e estratégias de gerenciamento do estresse.
