Pain management and its challenges in Iranian cancer patients: a mixed-method study

Objectives

To identify and explain the challenges of effective pain management in patients with cancer in Iran.

Design

A convergent mixed-methods study.

Setting

Oncology departments and palliative care units across multiple healthcare institutions in Iran.

Participants

Quantitative phase: 320 healthcare providers, including anaesthesiologists, general practitioners, oncologists, nurses and pharmacists, selected via convenience sampling. Qualitative phase: 10 stakeholders, including patients, caregivers, policy makers and clinicians.

Methods

Quantitative data were collected using a psychometrically validated 23-item questionnaire assessing knowledge, attitudes and perceived barriers to cancer pain management. Qualitative data were obtained through semistructured interviews and analysed using Graneheim and Lundman’s content analysis method with MaxQDA software. Integration was performed using a side-by-side approach.

Results

Quantitative data showed that over 65% of providers did not routinely assess pain, and only 29.1% believed pharmacological treatments were effective. Qualitative analysis identified 13 barriers across three domains—professional, patient and organisational—spanning physical, psychological, social and spiritual dimensions. Integrated findings revealed consistent patterns of underassessment, legal and cultural resistance and lack of interdisciplinary collaboration. These converging challenges highlight the need for holistic, system-level reform.

Conclusions

The convergence of quantitative and qualitative data reveals a multilayered system of barriers, professional, patient-related and organisational—rooted in physical, psychological, social and spiritual dimensions. These interlinked challenges contribute to fragmented pain management and limited interdisciplinary coordination. Addressing them requires a holistic reform strategy that integrates structural, cultural and clinical solutions.

Design and validation of the Disaster Health Literacy Questionnaire for diabetes patients in Iran: a mixed-methods study

Objectives

To develop and psychometrically evaluate a multidimensional Disaster Health Literacy Questionnaire (DHLQ) for diabetic patients in Iran, using advanced item response theory approaches. The questionnaire was designed in the Persian (Farsi) language.

Design

A sequential mixed-methods study incorporating qualitative (scoping review and interviews) and quantitative (psychometric validation) phases.

Setting

Diabetes clinics and healthcare centres across Iran (2022–2023).

Participants

The study enrolled 570 patients with diabetes (56% female, mean age 45.57±16.33 years) for quantitative validation; 15 experts and 15 patients for qualitative validation.

Outcome measures

The psychometric properties evaluated included content validity (using content validity ratio (CVR) and content validity index (CVI)), construct validity (assessed via multidimensional item response theory (MIRT)), and reliability (measured by Cronbach’s alpha and test-retest Kappa). Additionally, item parameters (multidimensional difficulty (MDIFF) and multidimensional discrimination (MDISC)) and model fit indices (RMSEA, CFI and TLI) were examined.

Results

The final 30-item DHLQ demonstrated excellent content validity (scale-level CVI=1; item-level CVI>0.79; CVR>0.49). Cronbach’s alpha for the total scale was 0.606; test-retest reliability showed significant agreement (Kappa=0.35–1, p

Conclusion

The DHLQ is a rigorously validated tool for assessing disaster health literacy in diabetic populations. Its multidimensional structure and strong psychometric properties support its use in clinical and emergency planning contexts to identify literacy gaps and tailor interventions.

Sociodemographic and clinical factors associated with non-adherence to stroke medication: an analytical, multi-hospital cross-sectional survey in Punjab, Pakistan

Objective

Studying issues related to stroke medication non-adherence is essential for secondary prevention of stroke. This study aimed to identify the prevalence of medication non-adherence and risk factors among stroke survivors. The reasons behind this are that some patients may not follow stroke medication plans, and potential ways to help patients adhere better to medication plans.

Design

This study employed a cross-sectional patient survey.

Setting

The study was conducted in 20 public and private healthcare facilities in a resource-constrained setting, in Punjab, the largest province of Pakistan.

Participants

We included 6538 stroke survivors aged 21–75 years with at least a 6 month history of stroke who were prescribed one or more anti-stroke medications and met the inclusion criteria.

Primary outcome measures

The main outcome was medication non-adherence, measured by the Self-Efficacy for Appropriate Medication Scale (SEAMS) and self-reported pill count. Descriptive statistics were used to summarise study variables. chi-square (²)/Fisher’s exact test and independent t-test/ANOVA were employed. A generalised linear model (logit model using multivariable logistic regression shows that several factors are associated with medication non-adherence and adherence. Odds ratio (OR) plots were generated using Seaborn and Matplotlib.

Results

Non-adherence based on pill counts was 49.7%, while the mean SEAMS score (31.3±7.7) showed moderate self-reported adherence. After adjusting for age, gender, marital status, education, income, health insurance, smoking status, comorbidities, stroke type, disease duration, blood pressure control, number of medications, dosing frequency, physiotherapy continuation, perceived side effects and doctor-patient satisfaction, we found that female gender (vs male: AOR 0.31, 95% CI 0.27 to 0.35), lower income (10k–25k PKR vs >100k PKR: AOR 0.31, 95% CI 0.23 to 0.41; 26k–50k PKR vs >100k PKR: AOR 0.57, 95% CI 0.47 to 0.68), primary/secondary education (vs postgraduate: AOR 0.74, 95% CI 0.64 to 0.87), controlled BP (vs uncontrolled: AOR 0.66, 95% CI 0.59 to 0.73), longer disease duration (≥5 years vs

Conclusion

This study addresses the significant issue of medication non-adherence in stroke patients in Pakistan, reflecting global patterns yet remaining under-explored locally. It emphasises the critical role of adherence in managing chronic conditions such as stroke, where consistent use of preventive therapies is vital for reducing recurrence and improving outcomes. While the non-adherence rates are consistent with global trends, there is a notable lack of observational studies and epidemiological data in the Pakistani context. Our findings support a comprehensive approach to enhance medication adherence, taking into account the complex connections among social, behavioural and clinical factors. It also highlights the importance of maintaining detailed records to monitor adherence trends, identify high-risk groups and inform targeted public health interventions.

Effect of foliar application of potassium on wheat tolerance to salt stress

Salinity stress severely hampers wheat productivity by impairing growth, photosynthesis, and metabolic balance. Potassium nutrition, however, can mitigate these effects by supporting physiological and biochemical stability. This study assessed the impact of foliar potassium application (0, 200 and 400 ppm) on two wheat cultivars, Galaxy-13 and Uqab-2000, exposed to normal (0 mM NaCl) and saline conditions (100 and 150 mM NaCl, respectively). Salinity significantly reduced root and shoot growth, biomass, chlorophyll content, photosynthetic rate, and stomatal conductance. Potassium supplementation, particularly at 400 ppm, alleviated these reductions, with Galaxy-13 showing a 32.01% increase in shoot length and a 45.11% increase in shoot dry weight compared to Uqab-2000. Biochemical analyses revealed that Galaxy-13 sustained higher nitrate and nitrite reductase activities (6.23 and 3.63 μmol NO2 g-1 FW h-1, respectively) and total soluble proteins (10.1 mg g-1 FW), whereas Uqab-2000 accumulated more soluble sugars and free amino acids under stress (9.8 and 19.8 mg g-1 FW, respectively). Oxidative stress indicators (malondialdehyde and hydrogen peroxide) rose under salinity, but potassium reduced their levels, with Galaxy-13 exhibiting stronger antioxidant regulation. Nutrient profiling further demonstrated that Galaxy-13 maintained higher N, P, and K contents and minimized Na uptake, unlike Uqab-2000, which showed severe ionic imbalance. Multivariate analyses (PCA, heatmap, and correlation) highlighted strong positive associations of potassium, especially K400, with biomass accumulation, photosynthetic efficiency, and nutrient homeostasis. The findings establish that Galaxy-13 possesses superior salinity tolerance and responds more favorably to potassium nutrition. This study provides novel evidence that cultivar-specific potassium management can enhance wheat resilience in saline environments, offering a practical strategy for sustaining yield under stress.

Characterising ocular injuries in competitive combat sports in Texas: a retrospective case-control study

Objective

This study aims to determine the incidence and impact of ocular injuries among the different combat sports disciplines of boxing, mixed martial arts (MMA), kickboxing and Muay Thai in Texas, USA.

Design

A case–control study was conducted to analyse retrospective postmatch physical reports from combat sports matches that took place in the state of Texas from January 2019 to January 2022. Ocular injuries and other match characteristics such as sport type and match outcome were identified by postmatch physical reports. Postmatch physical reports were collected from the Texas Department of Licensing and Regulation database. Statistical analysis was used to stratify injuries and compare the impact of injuries on match outcome.

Setting

Combat sports fighters in Texas, USA.

Participants

3070 participants were included in the study. Participants were fighters who participated in combat sports matches in Texas, USA, between January 2019 and January 2022.

Primary and secondary outcome measures

The original plan was to measure the incidence of ocular injuries across different combat sports including boxing, MMA, kickboxing and Muay Thai. However, due to a limited sample size of kickboxing and Muay Thai matches, the ocular injury incidence was only measured for boxing and MMA. The association between ocular injury and match outcome was assessed using 2 statistical analysis.

Results

The respective incidence rates of ocular injuries in boxing and MMA were 9.7 and 12.2 injuries per 100 matches. The association between ocular injury and match outcome (win, lose or draw) was statistically significant in boxing but not statistically significant in MMA matches.

Conclusions

Our findings revealed that ocular injuries are significantly associated to losing a boxing match (p=0.011), but not associated to match outcome in MMA (p=0.232). Additionally, MMA matches report a larger variety of ocular injuries compared with boxing matches.

The Effect of Theory‐Guided Positive Psychological Interventions for Patients With Breast Cancer: A Systematic Review and Meta‐Analysis

ABSTRACT

Aims

To critically assess the impact of theory-guided positive psychological interventions on the quality of life of breast cancer patients and survivors.

Design

Systematic review and meta-analysis.

Methods

A comprehensive literature search was conducted across seven electronic databases from inception to August 2024. Randomised controlled trials that examined the effects of theory-guided positive psychological interventions on adult breast cancer patients or survivors and reported quality of life outcomes were included. Screening, data extraction and critical appraisal were independently performed by the reviewers using the revised Cochrane risk-of-bias tool (RoB2). A meta-analysis was conducted using RevMan Web. The study was reported following the PRISMA 2020 Statement.

Results

Five randomised controlled trials were included. Only two of these studies showed a low risk of bias across all quality measures. The meta-analysis demonstrated a significant improvement in quality of life following theory-guided positive psychological interventions, with low heterogeneity. Subgroup analyses revealed that interventions lasting either less than or longer than 3 weeks, and both group-based and individual-based formats, had positive effects on quality of life. Additional psychological benefits were observed including reductions in depressive symptoms, anxiety, perceived stress and improvements in post-traumatic growth, resilience, hope and perceived benefits. One study also reported improvements in sleep quality.

Conclusion

The findings demonstrate that theory-guided positive psychological interventions can potentially significantly improve quality of life in breast cancer patients. However, the limited number of studies underscores the need for further high-quality research to validate these findings and identify the most effective intervention characteristics.

Relevance to Clinical Practice

Our systematic review highlights that theory-guided positive psychological interventions show promise as an effective strategy for improving the quality of life in breast cancer patients. These interventions can enhance psychosocial support strategies, paving the way for better-informed approaches that lead to improved patient outcomes.
