Buccal fat applied to transoral robotic lateral oropharyngectomy defects to lessen radical tonsillectomy pain (BOLT): a single-centre, phase II, parallel, randomised control trial study protocol

Introduction

Transoral robotic surgery (TORS) is a minimally invasive technique for surgical removal of tumours of the tonsil and lateral oropharynx. Surgical defects after TORS lateral oropharyngectomy are traditionally left open to heal by secondary intention, resulting in significant postoperative pain and secondarily resulting in delayed swallowing and discharge. Although multimodal analgesia can improve postoperative pain control, no studies to date have assessed the impact of adjunct surgical interventions for reducing postoperative pain after TORS. Buccal fat rotation flap is a regional reconstruction option after TORS lateral oropharyngectomy and provides immediate coverage of the open surgical wound. However, the impact of buccal fat rotation flap reconstruction on postoperative pain and swallowing remains unclear. This trial aims to compare postoperative pain outcomes in patients who undergo TORS lateral oropharyngectomy with and without buccal fat rotation reconstruction.

Methods and analysis

This protocol outlines a single centre, parallel, unblinded, phase II, randomised control trial. Inclusion criteria include adult patient (≥18 years) undergoing TORS lateral oropharyngectomy for early to intermediate stage tonsillar squamous cell carcinoma (T1-2N0-1 p16+/–) or early to intermediate stage salivary gland tumours of the palatine tonsils. Exclusion criteria include a history of prior head and neck squamous cell carcinoma, prior head and neck radiotherapy, retropharyngeal lymphadenopathy, bilateral lymphadenopathy, need for bilateral neck dissection, baseline trismus, opioid use or drug addiction, need for open surgery (transcervical lateral oropharyngectomy), free tissue transfer, or alternative regional flap, and pregnancy. All patients are planned for a TORS lateral oropharyngectomy. The intervention group will have a buccal fat rotation flap reconstruction, and the control group will be allowed to heal via secondary intention. The allocation sequence will be created using a computer-generated random sequence with a permuted block strategy. The allocation sequence will be concealed until the time of assignment. The primary outcome is postoperative pain intensity during rest and swallowing using the visualised analogue scale. Secondary outcomes include postoperative complications, other adverse events, patient-reported speech and swallowing, opioid usage, length of hospital stay, feeding tube dependence and blood glucose levels. The trial has a target sample size of 40 patients. Statistical analysis of the primary outcome will be analysed in an intention to treat analysis using a linear mixed effects model.

Ethics and dissemination

The study was approved by the University Health Network Coordinated Approval Process for Clinical Research. Study number CAPCR ID: 24-5894. All participants will be required to provide written informed consent to participate. Findings will be presented at national conferences and published in medical journals.

Self-management measurement instruments specific to individuals with cardiovascular diseases: a systematic review protocol using COSMIN methodology

Introduction

Cardiovascular diseases (CVDs) are the leading cause of death worldwide, making the development of self-management strategies crucial for preventing complications and improving clinical outcomes. This process involves symptom monitoring, treatment adherence, emotional management and a healthy lifestyle, among others. Reliable instruments are necessary to measure self-management, requiring robust psychometric properties. In this way, this COnsensus-based Standards for the selection of health Measurement INstruments (COSMIN)-based systematic review aims to assess the quality of specific self-management instruments for adults with CVDs.

Methods and analysis

This systematic review will follow the COSMIN and be reported according to the Preferred Reporting Items for Systematic Review and Meta-analysis Protocol. Searches will be conducted in seven databases: MEDLINE, Web of Science, Scopus, PsycINFO, EMBASE and CINAHL. Additionally, a manual search will be performed on PROQOLID, PROMIS and The Medical Outcome Trust websites. Studies on the development and validation of patient-reported instruments measuring specific self-management for individuals with CVDs will be included, without language or date restrictions. The search will be performed in November 2025, with the final version of the review expected to be completed in October 2026. Data extraction will follow COSMIN recommendations. The Modified Grading of Recommendations, Assessment, Development and Evaluation approach will be used to determine the quality of evidence. Instruments will be categorised according to COSMIN recommendations. All steps will be conducted by two independent reviewers, with a third reviewer involved in case of discrepancies. Additionally, the content of the instruments will be analysed and linked to the International Classification of Functioning, Disability and Health, following international recommendations.

Ethics and dissemination

This study does not require ethics committee approval as it is a review of published data. The review results will be disseminated through peer-reviewed journal publications and presentations at scientific conferences.

Reproduction of porcine ear necrosis (ear-tip necrosis) following intradermal inoculation of pigs with <i>Fusobacterium necrophorum</i>

Porcine ear necrosis (PEN) (also referred to as ear-tip necrosis, ETN) is a syndrome of global presence and unclear aetiology. Initially reported in the 1950s, many different infectious and non-infectious causes have been suggested as the causative(s) agent(s), but none has been confirmed in controlled studies. Here, we investigated the aetiology of PEN using pure culture of bacteria associated with lesions in controlled animal trials. A commercial farm with no history of ear-tip necrosis was identified and used as the source for 5-week-old pigs. Two independent trials were initially executed with identical designs. Piglets (=12/trial) were intradermally inoculated with either pure cultures of Staphylococcus hyicus or Fusobacterium necrophorum (left ear, n = 10) or sterile media (right ear, n = 10). Two pigs in each trial were not inoculated, serving as sentinels. A third trial used F. necrophorum as the inoculum, 3 pigs as sentinels and 9 as inoculated. All animals were clinically monitored daily following challenge, and an ear score was used to follow disease progression. All ears inoculated with S. hyicus remained lesion free. Four out of ten and 7/9 pigs challenged with F. necrophorum developed lesions undistinguishable from PEN, including necrosis and loss of portions of the ear pinna (P F. necrophorum was isolated from 4/10 and 7/9 pigs that developed necrotic lesions. Histopathology after resolution of necrosis revealed granulomatous tissue. Evidence presented here suggests that F. necrophorum causes PEN-like lesions, as seen in commercial barns. It is therefore suggested as the etiological agent of this syndrome.

Evaluating online information on temporomandibular disorders: readability, reliability, content and quality: a scoping review protocol

Introduction

Temporomandibular disorders (TMDs) are a prevalent group of musculoskeletal conditions affecting the temporomandibular joint, associated structures and masticatory muscles. The internet has become a primary source of health information for many patients; however, the readability, reliability, content and quality of online information on TMDs vary widely. A comprehensive synthesis of the characteristics and evaluation methods of such content is currently lacking.

Methods and analysis

This scoping review will follow the Joanna Briggs Institute methodology and be reported in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses extension for Scoping Reviews checklist. Peer-reviewed primary and secondary studies assessing online information on TMDs will be included if they report on readability, reliability, content or quality. Eligible information sources include publicly available websites, videos and social media; discussion forums and printed materials will be excluded. No language, date or geographical restrictions will be applied. A three-step search strategy will be implemented across PubMed, Web of Science, Embase, PsycINFO and CINAHL, followed by citation tracking. Screening will be conducted independently by two reviewers using Rayyan. Data will be extracted with a pilot-tested charting tool and synthesised narratively and descriptively in tabular and graphical formats.

Ethics and dissemination

As this study will only use data from publicly available sources, ethical approval is not required. Findings will be disseminated through publication in a peer-reviewed journal, conference presentations and professional networks, with the aim of guiding the development of accessible and reliable digital resources for individuals seeking information on TMDs.

Effectiveness and appropriateness of interventions to enhance the quality of life of informal caregivers of patients receiving home-based palliative care: a mixed-methods systematic review protocol

Introduction

Informal caregivers play a vital role in caring for individuals who choose to spend the end of their life at home. However, this caregiving role often imposes considerable physical, emotional, social and financial burdens that can negatively impact caregivers’ quality of life. A comprehensive understanding of the breadth of interventions designed to support caregivers of individuals receiving home-based palliative and end-of-life care is essential, along with insights into how these interventions are perceived by those who have received them. The objective of this review is to synthesise existing evidence on the effectiveness and appropriateness of interventions that support informal caregivers of patients receiving home-based palliative care in order to address the caregivers’ needs and improve their quality of life. Additionally, this review aims to explore the acceptability and perceived benefits of these interventions from the perspectives of informal caregivers who have received them.

Methods and analysis

A comprehensive search will be performed in the following databases: MEDLINE (PubMed), CINAHL, PsycINFO, Web of Science, Scopus, Cairn.info, EMBASE and the Cochrane Central Register of Controlled Trials (CENTRAL). This review will include studies that focus on adult informal caregivers of adult patients with serious life-threatening illnesses receiving home-based palliative care. Interventional studies that employed quantitative, qualitative and mixed-methods approaches will be considered. Quantitative studies will include randomised controlled trials (RCTs) and experimental and quasi-experimental designs. Qualitative studies will encompass research that explores informal caregivers’ experiences with the interventions, perceived benefits and barriers and enablers influencing intervention effectiveness. Mixed-methods studies using convergent, embedded or sequential designs will also be included.

The search will include studies published in English or French, with no restrictions on the publication period. Study selection, critical appraisal and data extraction will be conducted independently by two reviewers to ensure methodological rigour. This review will adhere to the Joanna Briggs Institute guidelines for mixed-methods systematic reviews, using a convergent segregated approach. Findings will be reported in accordance with the Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines.

Ethics and dissemination

Ethical review is not required for this study, as it is a literature review that does not involve the collection of primary data. The findings of this review will be disseminated to the scientific community through conference presentations and peer-reviewed publications. Additionally, lay summaries will be prepared and shared with the general public and relevant stakeholders.

História da institucionalização do cuidado de enfermagem em psiquiatria em uma cidade do nordeste do Brasil

A assistência psiquiátrica brasileira começou nas Santas Casas de Misericórdia até o surgimento dos hospitais psiquiátricos. Os objetivos deste estudo foram identificar vestígios do cuidado de enfermagem em psiquiatria nas primeiras instituições criadas no estado do Piauí, região Nordeste do Brasil e analisar suas relações com a institucionalização deste grupo na primeira metade do
século XX. Trata-se de um estudo qualitativo, histórico social, com dados publicados na mídia jornalística, coletados em arquivos públicos brasileiros. A análise, sob o referencial teórico foucaultiano, seguiu a metodologia de triangulação das fontes e interpretação de dados. Resultados: duas instituições iniciaram a assistência psiquiátrica no estado: o Asylo de Alienados, instituição pública criada em 1907, e o Sanatório Meduna, instituição privada inaugurada em 1954. Apesar do intervalo de 47 anos entre tais instituições, ambas instituíram o modelo manicomial, cuja prática assistencial incluía longos períodos de internação e tratamento disciplinar. Tal modelo abriu o mercado de trabalho para a enfermagem em psiquiatria, que se constituiu inicialmente de pessoas sem preparo formal, devido a inexistência de cursos de enfermagem no estado. Conclusão: o cuidado de enfermagem em psiquiatria foi institucionalizado no
Piauí/Brasil de acordo com a psiquiatria tradicional e a ausência de um saber próprio da enfermagem a colocou em condições de submissão ao poder médico, com poucos avanços na primeira metade do século XX.

Aspectos prácticos para la acogida humanizada en el centro quirúrgico y la sala de recuperación postanestésica: cuidando más allá de la técnica

La experiencia de un procedimiento quirúrgico puede ser angustiante tanto para los pacientes como
para sus familiares, lo que requiere la atención integral del equipo quirúrgico para minimizar las
inseguridades. Así, el objetivo de esta comunicación breve es reflexionar sobre la importancia de la
acogida humanizada en el Centro Quirúrgico y la Sala de Recuperación Postanestésica, presentando
aspectos prácticos para mejorar este enfoque. La humanización del cuidado implica una comunicación
efectiva, una atención personalizada y la creación de un ambiente acogedor. Este manuscrito destaca
estrategias para optimizar la acogida en el bloque quirúrgico. La humanización en la asistencia quirúrgica refuerza la esencia del cuidado en salud, yendo más allá de la mera aplicación de protocolos

Lucha antivenérea, bala mágica contra la avariosis en España

Sífilis enfermedad de difícil diagnóstico, en los albores del siglo XX, provocó un pánico social en España. Objetivo desvelar los avances en el tratamiento de la sífilis y descubrir la política sanitaria de lucha antivenérea, entre 1900 y 1953. Metodología a través del método heurístico se efectuó un análisis documental meticuloso tanto en fuentes directas como indirectas. Resultados, hacer frente al mal venéreo-sifilítico fue prioritario para las autoridades sanitarias españolas, debido al impacto de esta patología en el desarrollo económico del país. Por tanto, el descubrimiento casual en 1928, del Penicilium notatum supuso una revolución en el tratamiento de la sífilis. Sin embargo, las autoridades sanitarias preocupadas por la morbilidad de esta patología en plena Guerra Civil Española, impulsaron novedosos proyectos de salud pública, los cuales fueron un hito histórico. Conclusión gracias a la creación del cuerpo de Enfermeras Profesionales Visitadoras, el paciente sifilítico fue tratado de forma holística, confidencial y gratuita, hecho que asociado a las campañas de promoción de hábitos higiene sexual y educación sexual antivenérea que desarrollaron, logró que la tasa de mortalidad de la sífilis disminuyese. Luego, la lucha antivenérea ejecutada por estas enfermeras comunitarias, también puede ser considerada una bala mágica contra la sífilis.

Evidencias bibliográficas sobre riesgos laborales en los pescadores artesanales: revisión integrativa de la literatura

Objetivo: Identificar en la producción científica los tipos de riesgos para enfermidades y agravos para acidentes em trabajadores de la pesca artesanal. Metodología: Revisión integrativa, realizada entre enero y marzo de 2018, por medio de la combinación de los descriptores "Occupational Health" y "Fisheries", en las bases de datos, Medline, LILACS, Scopus, SciELO e internet. Se encontraron 123 artículos. Después de la lectura y evaluación de los mismos, se obtuvo un resultado final para el análisis de 16 artículos. Resultados principales: La mayoría de los estudios se indexaron en la base de datos Medline (56,3%), en el idioma inglés (43,8), con un nivel de evidencia VI (100%). Se constató que los trabajadores de la pesca artesanal están expuestos a riesgos ergonómicos, químicos, físicos y de accidentes. Conclusión: Se constató carencia de publicaciones en el área de la pesca artesanal, inexistencia de publicaciones científicas realizadas por la enfermería y brechas en el conocimiento.
