PREPARE—Empowering People Post Hip Fracture: A Conceptual Framework for a Nurse‐Led Model of Care. Results of a Modified e‐Delphi Study

ABSTRACT

Aim

To develop a person-centred nurse-led model of care framework to empower people post hip fracture.

Design

Modified e-Delphi study.

Methods

A Modified e-Delphi study seeking expert opinion from people with the lived experience of hip fracture and clinicians was implemented. Content experts consisted of 17 nursing and medical clinicians and four people with the experience of hip fracture.

Results

Study found > 70% consensus on all 20 statements rating the importance and feasibility of care components in the Modified e-Delphi survey. Themes developed from content analysis of expert free text responses comprised: Relationships support person-centred care; Value of a Specialist Hip Fracture Nurse; Prioritising is key to positive outcomes. PREPARE—Empowering People Post Hip Fracture: A Conceptual Framework for a nurse-led model of care was developed from study findings, highlighting key principles: person-centred care; evidence-informed practice; Health Empowerment; organisational governance; follow-up and evaluation, constituting this framework.

Conclusion

This study presents PREPARE—Empowering People Post Hip Fracture: A Conceptual Framework for a nurse-led model of care. PREPARE presents a structured approach to empowering people post hip fracture, outlining the aim and context in which the model of care is intended. It highlights an integrated, shared, coordinated approach to the care of people post hip fracture. Shared care empowers people and their support person to effectively manage their recovery journey and safely remain supported in the community.

Implications for Practice

PREPARE outlines a structured framework to support nurse leaders in implementing nurse-led models of care for people post hip fracture. There is an opportunity to empower nurse leaders and patients to support the recovery journey through education encompassing this person-centred holistic framework.

Impact

To our knowledge this is the first study to develop a conceptual framework for a nurse-led model of care to empower people post hip fracture. This model highlights opportunities for an integrated shared, coordinated approach to the care of people post hip fracture. PREPARE—Empowering People Post Hip Fracture: A Conceptual Framework for a nurse-led model of care, offers a structured approach for localised health service development of person-centred nurse-led empowerment models of care.

Factors Associated With the Survival of Older Patients With Pneumonia in the Emergency Department: A Retrospective Observational Study

ABSTRACT

Aims

To determine the clinical characteristics and identify not only the Korean Triage and Acuity Scale levels of older patients with pneumonia in the emergency department but also the factors associated with their survival.

Design

This study employed a retrospective observational design.

Methods

This study was conducted at the emergency department of a university hospital in Seoul, South Korea. It utilised medical data from January 1 to December 31, 2023. The study sample comprised 327 patients aged 65 years or older who received a pneumonia diagnosis (International Classification of Diseases: J10–J18). Binary logistic regression analysis was performed to identify independent factors associated with their survival.

Results

Survival was significantly associated with and influenced by sex (specifically, male sex), initial Korean Triage and Acuity Scale level, oxygen supplementation in the emergency department, consciousness level (specifically, painful response), body temperature (> 37.5°C) and a diagnosis of solid or hematologic malignancies.

Conclusions

The findings highlight the need for improved triage protocols, emphasising consciousness level, body temperature and malignancies. Incorporating geriatric-specific age thresholds and oncologic status into Korean Triage and Acuity Scale classifications may enhance risk stratification, timely intervention and resource allocation in emergency department settings.

Implications for the Profession and/or Patient Care

This study provides insight into triage accuracy for older pneumonia patients, emphasising early recognition of high-risk individuals and strengthening nursing assessment protocols. Improved Korean Triage and Acuity Scale classifications can optimise resource allocation and emergency care strategies, ultimately reducing mortality rates.

Impact

The study provides actionable insights for emergency nurses, triage clinicians and policymakers. The findings support the refinement of KTAS protocols to enhance risk stratification and guide resource allocation for older pneumonia patients, ultimately aiming to reduce mortality rates.

Exploring the Service Features of Telephone Cancer Information and Support Services From Callers' Experiences: A Qualitative Evidence Synthesis

ABSTRACT

Aim

This study aims to synthesise evidence on users' experiences of telephone cancer information and support services (CISS) to identify important service features and inform service development.

Design

A qualitative evidence synthesis.

Methods and Data Sources

OVID MEDLINE, EMBASE, CINAHL, PsycINFO and SocINDEX databases were searched for peer-reviewed qualitative literature fitting the inclusion criteria from database inception to 30 March 2023. The included articles were double-screened, and quality appraised using the CASP checklist. GRADE-CERQual was used as a tool to assess the confidence of review findings. Content synthesis combined the qualitative data with the Loiselle cancer experience measurement framework guiding analysis. This paper is reported as per the equator network recommended SRQR checklist.

Results

Of the 607 articles screened, seven studies were included. Four main themes and 14 sub-themes about CISS aspects were identified: psychological well-being (managing emotions/coping, hope/reassurance, supporting close others and a reluctance to call the CISS); knowledge is power (information seeking, the burden of knowledge and empowerment); truth and clarity (adjunct support, credible source and improved understanding and confidence); and service adequacy (operators' ability to connect with users, convenience, service provision and awareness, and the cancer journey).

Conclusion

Findings suggest people with cancer and their carers accessing a CISS value emotional support combined with trusted information, topic expertise and a connection with the service operator. future service provision should address the lack of awareness regarding the range of services and the convenience extended operating hours may offer.

Implications for Patient Care

The results add to our understanding of CISS service provision. However, knowledge gaps remain regarding preferences among service features and the hierarchy of CISS characteristics to be prioritised to enhance services.

Impact

Focused CISS awareness campaigns will inform communities and healthcare professionals of the available resources to improve the lives of those affected by cancer. Ongoing service review will enable resources to be tailored to callers' needs, potentially easing the burden on existing services that are overwhelmed and under-resourced.

Carrying the Unspoken: A Phenomenological Study of the Existential Experiences of Siblings of Children With Complex Care Needs

ABSTRACT

Aim

To explore the existential lived experiences of emerging adult siblings of children with complex care needs.

Design

A qualitative phenomenological design.

Methods

In-depth conversational interviews were conducted between February and June 2022 with nine emerging adult siblings (aged 16–27), who grew up with a brother or sister with complex care needs. Data were analysed using van Manen's phenomenology of practice approach.

Results

Five core themes were identified: Loss of a familiar world: the profound changes and disruptions in siblings' lives. The sibling bond: endured and enduring love. Embracing the load: balancing responsibilities. Being behind the scenes: a lonely childhood. Jigsaw falling into place: siblings finding clarity and forging their own paths.

Conclusion

Early experiences of growing up with a sibling who has complex care needs, may resurface or shift in meaning, impacting long-term existential well-being, particularly when siblings feel overlooked by families, educators, and healthcare professionals. These silenced experiences often persist into emerging adulthood, shaping emotional health, relationships, and life choices.

Impact

The underrecognized existential needs of these siblings call for intentional, person-centred care. Our research advocates early, targeted interventions, emphasizing the pivotal role of nurses. A lifeworld-led phenomenological approach equips nurses to more effectively attend to the unmet needs of siblings within family care settings.

The research followed the Consolidated Criteria for Reporting Qualitative Research (COREQ) checklist.

Emerging adult siblings with lived experience of growing up alongside a brother or sister with complex care needs contributed important insights into the interpretation of findings, ensuring relevance to clinical nursing.

Validity and Reliability of the Stoma Self‐Efficacy Scale in Patients With Intestinal Stoma: A Descriptive, Cross‐Sectional and Validation Study

ABSTRACT

Aim(s)

The aim was to validate the stoma self-efficacy scale and assess the psychometric properties of the Czech version of the scale in patients with intestinal stoma. Another aim was to assess self-efficacy in patients with intestinal stoma.

Design

Descriptive, cross-sectional and validation study.

Methods

Two hundred and ninety patients with intestinal stoma participated in the study during 2023. The original SSES instrument was linguistically validated into Czech. Content validity of the scale, test–retest, intraclass coefficient, Cronbach's alpha, McDonald's ω, construct and convergent validity were assessed for psychometric properties. The study followed STROBE guidelines.

Results

The stoma self-efficacy scale was adopted into Czech, demonstrating excellent content validity. An intraclass correlation coefficient was calculated to establish test–retest reliability, showing excellent reliability of the Czech version. Cronbach's alpha and McDonald's ω showed high reliability. Factor analysis was applied for construct validity. Exploratory factor analysis was used to extract three factors on the Czech version of the scale: Stoma care self-efficacy, social self-efficacy and burden self-efficacy. The factors accounted for 62.05% of the total variance and showed strong internal consistency. Confirmatory factor analysis was applied separately to the data of respondents with colostomy and respondents with ileostomy. The fit indices were satisfactory for respondents with colostomy after adjustment. The composite reliability coefficient showed acceptable values in each factor.

Conclusion

The Czech version of the stoma self-efficacy scale has excellent psychometric properties in patients with intestinal stoma. It is a reliable tool for use in patients with intestinal stoma to assess self-efficacy. The scale can also be used by nurses who care for these patients and based on this, meet the individual needs related to patients' self-efficacy.

Generative AI at the Bedside: An Integrative Review of Applications and Implications in Clinical Nursing Practice

ABSTRACT

Aim

The aim of this integrative review is to critically appraise and synthesise empirical evidence on the clinical applications, outcomes, and implications of generative artificial intelligence in nursing practice.

Design

Integrative review following Whittemore and Knafl's five-stage framework.

Methods

Systematic searches were performed for peer-reviewed articles and book chapters published between 1 January 2018 and 30 June 2025. Two reviewers independently screened titles/abstracts and full texts against predefined inclusion/exclusion criteria focused on generative artificial intelligence tools embedded in nursing clinical workflow (excluding nursing education-only applications). Data were extracted into a standardised matrix and appraised for quality using design-appropriate checklists. Guided by Whittemore and Knafl's integrative review framework, a constant comparative analysis was applied to derive the main themes and subthemes.

Data Sources

CINAHL, MEDLINE, and Embase.

Results

Included literature was a representative mix of single-group quality improvement pilots, mixed-method usability and feasibility studies, randomised controlled trials, qualitative descriptive and phenomenological studies, as well as preliminary and proof-of-concept observational research. Four overarching themes emerged: (1) Workflow Integration and Efficiency, (2) AI-Augmented Clinical Reasoning, (3) Patient-Facing Communication and Education, and (4) Role Boundaries, Ethics and Trust.

Conclusion

Generative artificial intelligence holds promise for enhancing nursing efficiency, supporting clinical decision making, and extending patient communication. However, consistent human validation, ethical boundary setting, and more rigorous, longitudinal outcome and equity evaluations are essential before widespread clinical adoption.

Implications for the Profession and Patient Care

Although generative artificial intelligence could reduce nurses' documentation workload and routine decision-making burden, these gains cannot be assumed. Safe and effective integration will require rigorous nurse training, robust governance, transparent labelling of AI-generated content, and ongoing evaluation of both clinical outcomes and equity impacts. Without these safeguards, generative artificial intelligence risks introducing new errors and undermining patient safety and trust.

A Mixed‐Methods Exploration of Staff Needs for Coping With Grief and Loss in Residential Aged Care

ABSTRACT

Aims

To examine residential aged care staff's experience of death and grief, and their support needs.

Methods

A mixed-methods sequential explanatory design, using an online cross-sectional survey that included the Texas Revised Inventory of Grief and the Grief Support in Health Care Scale. Followed by semi-structured interviews with direct care workers and managers working in residential aged care homes were conducted.

Results

Over 60% of participants experienced five or more resident deaths in the previous 12 months. Although, different levels of grief were experienced among different roles, the importance of open communication and opportunities for farewells after resident death was highlighted. Participants suggested support and education to normalise grief and promote self-care.

Conclusion

Recognising staff grief following the resident death is important. Providing support and education may help improve staff wellbeing and contribute to the delivery of high-quality care for both residents and their families.

Implication for the Profession and/or Patient Care

Staff grief after a resident death needs to be recognised, and continuing education and support are required for their wellbeing.

Beyond clinical trials: Extending the role of the clinical research nurse into social care and homeless research

Abstract

Aim

Clinical research nurses work at the fulcrum of clinical trials with clearly defined roles and responsibilities. In England, the National Institute for Health Research (the main funder of health research) has broadened its scope to encompass social care research. The expectation is that clinical research nurses will expand their skill set to support these new studies, many of which will employ qualitative and mixed methods. This discussion paper explores the challenges of facilitating this clinical academic workforce development through a case study of a homeless health and social care research project. This was one of the first studies to engage clinical research nurses in this new and expanded role.

Background

Much of what is known about the research nurse workforce has been generated through studies of clinical trials in oncology. The ‘caring-recruiting’ dichotomy has been used as a heuristic device for identifying workforce issues that can impact on study delivery such as how intense pressure to recruit study participants leads to low job satisfaction.

Design

This case study reflects on the authors' experiences of employing a clinical research nurse in a social care research project concerned with the discharge of homeless people from hospital. The ‘caring-recruiting’ dichotomy is used to generate new information about the relationship between workforce development and the successful delivery of social care research.

Conclusion

The case study illuminates how social care research can generate different pressures and ethical challenges for research nurses. The time and skill it took to recruit study participants identified as ‘hard to reach’ was suggestive of the need to move beyond performance measures that prioritise recruitment metrics. The need for different types of staff supervision and training was also warranted as supporting study participants who were homeless was often distressing, leading to professional boundary issues.

Relevance to workforce development

This study highlights that performance management, training and supervisory arrangements must be tailored to the characteristics of each new study coming onto the portfolio to ensure research nurses are fully supported in this new and expanded role.

Nursing students’ experience and training in healthcare aid during the COVID‐19 pandemic in Spain

Background

During the COVID-19 pandemic health crisis, in some countries such as Spain, nursing students have offered to provide health assistance, but the role they have played, their degree of preparedness to face the situation, and what must be improved in their training to be ready for these situations is unknown.

Objective

Describe the experience and perceptions of students of the Nursing university degree during their participation as health support in the COVID-19 health crisis in Spain.

Design

We conducted a cross-sectional survey study.

Method

503 students enrolled in the 4th course of the Nursing degree in Spain during the months of March and April 2020. An online questionnaire was developed, based on a pilot study and distributed through the Nursing Association, students’ unions and students’ associations. Variables were used to describe their participation, degree of preparedness and training needs to determine how to improve training through descriptive statistics, as well as nonparametric tests to analyse the relationship between training and degree of preparedness nursing students. Results are reported according to the STROBE Statement.

Results

73.2% (368) of students offered to participate in healthcare aid, of which 225 were actively involved. 27.8% carried out nursing tasks without supervision, and 47.7% assisted COVID-19 patients as any other nurse. Only 3.4% felt very prepared to work in the field of intensive care, finding that those students who perceived a higher degree of preparedness had received previous training in personal protective equipment and mechanical ventilation (p < 0.005). The highest scores for training activities that may improve their preparedness were simulations to improve levels of anxiety and stress when managing critical patients, simulation in ventilatory support and mandatory practices in services where ventilators are used.

Conclusions

Although three out of four students were willing to provide health assistance, they recognise that they were not specially prepared in the field of intensive care and demand training with simulation to improve anxiety and stress levels in the management of critical patients and simulation in ventilatory support.

Relevance to clinical practice

Students have been vital resources for our health system and society when they have been needed. It is now up to us, both teachers and health authorities, to share their efforts by implementing the necessary improvements in training and safety measures not only because these affects the health and safety of the patient, but because they will be essential parts in future pandemics.

A grounded theory of clinical nurses’ process of coping during COVID‐19

Abstract

Aims and Objectives

To explore clinical nurses’ process of coping during COVID-19 and develop a grounded theory that can be used by leaders to support clinical nurses during a disaster.

Background

The COVID-19 pandemic has provoked widespread disruption to clinical nurses’ work. It is important to understand clinical nurses’ processes of coping during disasters to support the nursing workforce during events such as global pandemics.

Design

We employed the Corbin and Strauss variant of grounded theory methodology, informed by symbolic interactionism, and applied the EQUATOR guidelines for qualitative research publication (COREQ).

Methods

Data collection entailed semi-structured interviews with experienced clinical nurses (n =20) across diverse settings. We analysed data by identifying key points in the nurses’ coping processes inductively building concepts around these points.

Results

The predictor of nurses’ outcomes in this grounded theory was their confidence in their ability to cope during the pandemic. When nurses lacked confidence, they experienced working in the context of acute COVID—a state of chaos and anxiety, with negative consequences for nurses. However, when nurses were confident in their abilities to cope with the pandemic, they experienced working in the context of chronic COVID, a calmer state of acceptance. There were many workplace factors that influenced nurses’ confidence, including adequacy of personal protective equipment, clear information and guidance, supportive leadership, teamwork and adequate staffing.

Conclusions

Understanding clinical nurses’ experience of coping during COVID-19 is essential to maintain the nursing workforce during similar disasters.

Relevance to clinical practice

Nurse leaders can target areas that support nurses’ confidence, such as adequate PPE and staffing. In turn, increased confidence enables clinical nurses to cope during disasters such as a global pandemic.

Building capacity in times of crisis: Increasing cultural competence of healthcare professionals in the context of the COVID‐19

Background

Nurses are the frontline healthcare professionals fighting the medical and social effects of the current COVID-19 pandemic. Although they work with diverse populations, there is a lack of literature on culturally competent education during an emergency such as a pandemic.

Aims and objectives

To examine the effectiveness of an online education programme aimed at increasing cultural competence among rescue teams and healthcare professionals facing the challenges of the COVID-19 pandemic.

Design

Pre–post-web-based intervention study.

Methods

Pre–post-intervention surveys were administered to a matched sample of Israeli healthcare and rescue organisation professionals. The initial sample included 303 participants (52% women) who completed the pre-intervention survey. More than half of the sample (56%, n = 170) were paid workers or volunteers in health organisations. Of the initial sample, 154 participants completed the post-intervention survey following the online cultural competence education programme. Descriptive statistics and multivariate analysis were used to evaluate participants’ gains in culturally competent attitudes, knowledge, skills and encounters. This study followed the STROBE guidelines.

Results

Participants found the online programme useful in improving their cultural competence during the COVID-19 pandemic. The highest gains were found in the attitudes domains, whereas the lowest in the knowledge domain. Pre-intervention cultural competence scores and incorporating the programme in the educational curricula predicted increased gains in cultural competence.

Conclusions

Online educational interventions showed potential for increasing professionals’ awareness of cultural biases, differences and attitudes, leading to more open and accepting attitudes towards patients of different backgrounds.

Relevance to clinical practice

Recognising the need for real-time, low-cost and available training, the World Health Organization recommended using online courses for healthcare professionals struggling in the pandemic frontline. Online education programmes provide a useful platform for training health professionals in times of emergency.

A unique disaster response in aged residential dementia care: Can the experience inform future care models?

Abstract

Aims and Objectives

To understand how staff who chose to live-in with residents in a level 3 dementia care unit perceived the experience, in particular, their perceptions of how residing on site affected resident well-being.

Background

COVID-19 has been especially devastating in aged residential care (ARC) facilities. In March 2020, when the threat became realised in New Zealand, one residential dementia care facility implemented a unique response to the imminent threat of COVID-19. Eight staff members made the decision to live on site during the lockdown, ensuring residents’ risk of contracting the virus was significantly reduced as carers would not go outside of the facility.

Design

A qualitative descriptive inquiry.

Methods

Seven staff who chose to live-in, and the facility manager, participated in semi-structured, face-to-face interviews at the ARC. Audio-recorded interviews were transcribed verbatim and analysed using a thematic analysis approach. COREQ guidelines were adhered to in the reporting of this study.

Results

An overarching motif which emerged from the findings was the articulation of an ‘all in this together’ attitude which fostered feelings of camaraderie and collaboration which enhanced the experience for staff individually, and as a group. Themes identified were as follows: (a) A ‘safe’ but challenging choice, (b) Benefits for the staff and (c) Positive outcomes for the residents.

Conclusion

This crisis inadvertently brought about an enhanced ‘dementia-friendly’, person-centred communal environment.

Relevance to clinical practice

This study identified themes that deepen our understanding of caring for vulnerable populations during a pandemic and beyond. Given the success of this ‘live-in’ innovation, consideration must be given to applying these findings more generally when planning care models for best outcomes for residents receiving rest home level dementia care. How we care for people in disaster situations reflects the heart of the caring workforce, but such innovation may be extended to usual care where indicated.

Examining the role of micronutrients on improving long COVID sleep‐related symptoms

Abstract

Aims and Objectives

Long COVID is defined as the continuation of symptoms for four or more weeks after initial contraction of the virus. This review article examines the role of four select micronutrients (zinc, vitamins C, D and polyphenols) for their anti-inflammatory and therapeutic potential to improve sleep-related symptoms in persons with long COVID.

Background

Evidence suggests a link between long COVID and increased inflammation. There are currently no therapeutic interventions for common sleep-related symptoms associated with long COVID. Micronutrients, due to their antioxidant and anti-inflammatory properties, may have a role in the treatment of sleep-related symptoms in the context of long COVID.

Design

A narrative literature review was conducted and guided by the PRISMA checklist.

Methods

All articles were screened from PubMed, ScienceDirect, NCBI or Google Scholar and were limited to human studies. The following keywords were used: ‘COVID-19’, ‘sleep symptoms’, ‘zinc’, ‘vitamin C’, ‘vitamin D’, ‘polyphenols’ and ‘micronutrients’.

Results

There are currently no studies that examine the usage of micronutrients and its impacts on long-term, sleep-related symptoms post-COVID-19 infection. We focussed our review on prior studies that examined micronutrients in the context of sleep symptoms and inflammation, while exploring the potential for micronutrients to help improve sleep-related symptoms associated with long COVID.

Conclusions

There is evidence to suggest that sleep-related symptoms associated with long COVID, such as fatigue and poor sleep quality, are associated with inflammation. Zinc, vitamins C, D and polyphenols all have the potential to improve both inflammation and sleep quality to alleviate symptoms. Future research should further examine these micronutrients in the context of long COVID to improve sleep and quality of life.

Relevance to Clinical Practice

This article provides implications for clinicians to be at the forefront of research on the usage of micronutrients to improve sleep-related symptoms in persons with long COVID.

Psychological trauma among nurses during the COVID‐19 pandemic with strategies for healing and resilience: An integrative review

Abstract

Aims and Objectives

The aim of this study was to identify nurses' responses to psychological trauma and strategies to support nurses' healing and resilience during COVID-19 and generate creative integrated understandings of nurses' responses to psychological trauma and strategies supporting nurses' healing and resilience during COVID-19.

Background

COVID-19 exacerbated trauma already experienced by some nurses. Nursing leadership called for action to improve nurses' mental health and resilience. However, policy changes have been rudimentary and insufficiently funded. Negative impacts manifesting as mental health disorders may significantly disrupt care quality, deepen nursing shortages and de-stabilize healthcare systems. Building nurses' capacity to respond with resilience is widely indicated for countering harmful effects of psychological trauma and enabling professional longevity.

Design

Integrative review design was used to support discovery of emergent knowledge, as phenomena of interest lacked a traditional empirical evidence base.

Methods

Cumulative Index to Nursing and Allied Health, ProQuest Nursing & Allied Health, and PubMed databases were searched for nursing publications, January–October 2020. Search words included nurs*, COVID-19, Coronavirus, pandemic, post-traumatic stress disorder, trauma, mental health, resilience. PRISMA Checklist standards guided reporting. Joanna Briggs Institute tools facilitated quality measurement. Inclusion criteria were English language and nursing focus on trauma, healing or resilience strategies. Thirty-five articles met inclusion criteria. Elo and Kyngäs' qualitative content analysis method guided thematic analysis.

Results

Findings suggest dysfunctional responses for some nurses to COVID-19 trauma, or living fearful, uncertain and unstable. Findings also reveal numerous potential regenerative healing and resilience strategies for nurses, or living whole, optimistic and supported. Workforce management and patient outcomes in the intensive care unit during the COVID‐19 pandemic and beyond: a discursive paper

Abstract

Aims

To highlight the need for the development of effective and realistic workforce strategies for critical care nurses, in both a steady state and pandemic.

Background

In acute care settings, there is an inverse relationship between nurse staffing and iatrogenesis, including mortality. Despite this, there remains a lack of consensus on how to determine safe staffing levels. Intensive care units (ICU) provide highly specialised complex healthcare treatments. In developed countries, mortality rates in the ICU setting are high and significantly varied after adjustment for diagnosis. The variability has been attributed to systems, patient and provider issues including the workload of critical care nurses.

Design

Findings

Nursing workforce is the single most influential mediating variable on ICU patient outcomes. Numerous systematic reviews have been undertaken in an effort to quantify the effect of critical care nurses on mortality and morbidity, invariably leading to the conclusion that the association is similar to that reported in acute care studies. This is a consequence of methodological limitations, inconsistent operational definitions and variability in endpoint measures. We evaluated the impact inadequate measurement has had on capturing relevant critical care data, and we argue for the need to develop effective and realistic ICU workforce measures.

Conclusion

COVID-19 has placed an unprecedented demand on providing health care in the ICU. Mortality associated with ICU admission has been startling during the pandemic. While ICU systems have largely remained static, the context in which care is provided is profoundly dynamic and the role and impact of the critical care nurse needs to be measured accordingly. Often, nurses are passive recipients of unplanned and under-resourced changes to workload, and this has been brought into stark visibility with the current COVID-19 situation. Unless critical care nurses are engaged in systems management, achieving consistently optimal ICU patient outcomes will remain elusive.

Relevance to clinical practice

Objective measures commonly fail to capture the complexity of the critical care nurses’ role despite evidence to indicate that as workload increases so does risk of patient mortality, job stress and attrition. Critical care nurses must lead system change to develop and evaluate valid and reliable workforce measures.

COVID‐19 preparedness in aged care: A qualitative study exploring residential aged care facility managers experiences planning for a pandemic

Abstract

Aims and Objectives

The study aims to understand the changing context of RACFs and the role of RACF managers in preparing to confront the COVID-19 pandemic and to provide insights into how the use of visual telehealth consultation might be incorporated to assist with managing whatever might arise.

Design

An interpretive descriptive study design was employed, and data were collected using semi-structured interviews conducted via telephone or videoconference. Purposive recruitment targeted clinical managers responsible for the COVID-19 response in RACFs.

Methods

RACF clinical managers were invited to discuss their responses to COVID-19 including the management of RACF and staff. Semi-structured interviews explored the COVID-19-related challenges, the response to these challenges and how telehealth might assist in overcoming some of these challenges. This study followed Thorne's (2008) three-stage process of interpretive description. The COREQ checklist was used in preparing this manuscript.

Results

Two main themes were identified. The first theme ‘keeping people safe’ was comprised of three subthemes; fear and uncertainty, managing the risks and retaining and recruiting staff. The second theme was ‘keeping people connected’, had two subthemes; being disconnected and isolated and embracing technology.

Conclusion

Findings from this study provide valuable insight into understanding the context and the challenges for RACFs and the staff as they attempt to keep residents safe and connected with healthcare providers and the outside world.

Relevance to Clinical Practice

Understanding the experiences of RACF managers in preparing to respond to the pandemic will better inform practice development in aged care in particular the use of telehealth and safe practices during COVID-19. Increased awareness of the challenges faced by RACFs during a pandemic provides policymakers with valuable insights for future planning of pandemic responses.

Development and Evaluation of Precision Health Competencies

ABSTRACT

Aim

To develop precision health (PH) competencies and evaluate their comprehensiveness and fit into nursing practice.

Design

A modified e-Delphi technique was used to gather perceptions and achieve consensus on the inaugural set of PH domains, competency statements and sub-competencies developed by a workgroup formed under the aegis of the American Nurses Association (ANA).

Methods

A set of PH competencies and sub-competencies was developed by the ANA workgroup, beginning with a literature review, followed by a multi-step work process of the group over 3 years (2022–2025). Then, a modified e-Delphi technique was conducted via a four-point Likert scale Qualtrics survey, using a purposive sample of PH experts. The respondents were asked to agree or disagree with each competency or sub-competency statement and suggest modifications. The threshold of concordance was set at 80%.

Results

The ANA workgroup reached consensus on six domains, six competency statements and 43 sub-competency statements to represent PH in nursing practice in its entirety. Forty experts in the field evaluated and offered revisions to the final 44 sub-competencies that represent the knowledge and skills necessary for PH in general nursing practice. A majority of the competency statements obtained favourable agreement from the expert panel, and a typical pattern of convergence was observed over two rounds of evaluation.

Conclusions

The development of PH competencies is the essential first step in the attempt to integrate PH into nursing practice.

Implications

The competency statements will inform nursing curricula, clinical practice guidelines, funding opportunities and role expectations in all healthcare settings.

Impact

This work sets the stage for subsequent interprofessional practice initiatives and research exploring how these competencies influence patient outcomes, workforce readiness and the practical integration of advanced technologies into precise care.

Communication Between Patients and Healthcare Professionals in Neurological Hospitalisation: A Qualitative Photo‐Voice Study

ABSTRACT

Aim

To explore and compare patients' and healthcare professionals' experiences of communication during hospitalisation for neurological diseases.

Background

Effective communication is essential for establishing strong relationships between patients and healthcare professionals, thereby ensuring patient-centred care that respects individual values and preferences. Neurological patients face additional communication challenges due to cognitive and motor deficits, such as speech difficulties and delayed cognitive processing. Limited research has investigated how both patients and healthcare professionals experience communication in this context.

Design

An explorative, qualitative design was applied within a hermeneutic framework inspired by photo-voice methods.

Methods

Data were obtained through interviews with patients (n = 12), one focus group discussion with healthcare professionals (n = 10) and six additional interviews with healthcare professionals (n = 6). Interviews were combined with photographs taken during the interviews. Data were analysed using reflexive thematic analysis, and the COREQ guideline was applied.

Results

The analysis revealed a main theme: The core of connected care, encompassing three subthemes: Guided alignment, A changing environment and Human before patient. These themes created the foundation for effective, compassionate and humanised care. Participants metaphorically compared this to an aquarium, emphasising that, like an ecosystem, effective communication requires balance between alignment, environment and humanity. This main theme represents the quality of communication between patients with cognitive challenges and their healthcare providers.

Conclusions

This study provides insight into the experiences of communication from both patients and healthcare professionals. Effective communication is important to manage treatment and engage patients in care.

Implications for Practice

Improving communication, promoting shared decision-making and enhancing the implementation of person-centred care are key strategies for increasing patient outcomes and satisfaction.

The Effect of Artificial Intelligence in Promoting Positive Nursing Practice Environments: Mixed Methods Systematic Review

Aim

To synthesise the available evidence on the effect of artificial intelligence in promoting positive nursing practice environments, exploring outcomes for professionals, clients, and institutions.

Background

Artificial intelligence has undergone significant advancements and shows great potential to transform nursing practice. However, this technological evolution is not without challenges, which must be identified and addressed.

Methods

A systematic mixed-methods review following the PRISMA 2020 guidelines and the methodology proposed by JBI. The search strategy was conducted in PubMed, CINAHL, Scopus, and Web of Science, including grey literature. Quantitative, qualitative, and mixed-methods studies were included, and the selection process involved screening by two independent reviewers, who assessed all studies, their methodological quality and extracted their data.

Results

From the conducted search, 11 studies were included, addressing how artificial intelligence has transformed nursing practice by optimising decision-making, task execution, and patient safety. Artificial intelligence, through predictive models and assistants such as ChatGPT, can enhance nursing management. However, challenges such as privacy concerns, resistance to change, and the need for professional training must be addressed to maximise its effectiveness.

Conclusion

Artificial intelligence has the potential to positively transform the nursing practice environment, optimising decision-making, enhancing patient safety, and improving operational efficiency, with clear benefits for professionals, patients, and healthcare institutions.

Relevance to Clinical Practice

This study analysed the impact of artificial intelligence on nursing, highlighting improvements in clinical decision-making, patient safety, and institutional efficiency. Despite the identified benefits, the implementation of artificial intelligence in nursing is not without challenges and risks, which must be identified and addressed to ensure safe and effective adoption.

Research Capability and Influencing Factors Among Clinical Nurses: A Multicentre Cross‐Sectional Study

ABSTRACT

Aim

To evaluate the research capability of clinical nurses in China and identify the determinants associated with their capability.

Background

As nursing evolves into an increasingly independent discipline, the research capability of clinical nurses has become critical for the development of the profession, advancing evidence-based practice and improving patient care quality.

Methods

A multicentre cross-sectional survey was conducted using convenience sampling from September 2023 to February 2024, among clinical nurses in tertiary hospitals across three provinces in China. The Nursing Research Capability Self-Assessment Scale was used to assess the research capability of the nurses. Chi-square tests, one-way analysis of variance and multiple linear regression were used to examine factors associated with research capability. The Strengthening the Reporting of Observational Studies in Epidemiology was followed.

Results

A total of 1074 clinical nurses participated. The mean research capability score was 89.11 ± 27.69, reflecting a moderate level of research capability. However, two dimensions of research questions and literature review received lower scores. Multiple linear regression analysis identified that education level, professional title, administrative position and nursing job title (all p < 0.05) were independent predictors of research capability.

Conclusions

Clinical nurses exhibit moderate research capability, with notable deficiencies in formulating research questions and conducting literature reviews. Key factors influencing research capability include education, professional title, administrative position, and job title. Targeted training and development programmes should address these factors to enhance nurses' research competence and advance nursing science.
