To conceptualise information distortion in Electronic Health Records (EHRs), with the goal of providing a theoretical foundation for improving documentation practices.
A concept analysis.
Walker and Avant's strategy for concept analysis was used. The defining attributes, antecedents and consequences were identified.
A comprehensive search was conducted across PubMed, Web of Science, Embase, CINAHL and Scopus from their inception to December 2024. Studies published in English that addressed information distortion in EHRs were included.
A total of 37 studies were included. The three defining attributes were: real-world health truth, representation of reality and mismatch relationship. Antecedents were divided into five categories: people-related factors, equipment factors, regulatory factors, working environment factors and management factors. The consequences of information distortion in EHRs included threats to patient safety, poor operational performance, eroded trust, compromised research quality and health inequity.
This concept analysis enhances the understanding of information distortion in EHRs and provides a foundation for further empirical validation. The findings may contribute to the development of measurement instruments and strategies to mitigate information distortion in healthcare settings.
By undertaking a concept analysis of information distortion in EHRs, healthcare professionals will be better equipped to recognise and assess this ethical phenomenon, thereby supporting the development of targeted interventions to mitigate potential harms to healthcare practices. In addition, the clarity of this concept could provide a new angle from which to analyse the origins of flawed EHR documentation and its ripple effects across healthcare systems.

Diabetic foot wounds (DFW) are notoriously difficult to treat owing to poor vascularity, delayed healing and higher rates of infection. Human-derived acellular dermal matrices (ADM) have been used in DFW treatment, utilizing a matrix scaffold for new tissue generation. We investigate the efficacy of a micronized injectable human-derived ADM in the treatment of DFW. We retrospectively recruited 13 patients with diabetic foot wounds. Wounds were adequately debrided, and a micronized injectable ADM was applied. Wound sizes were recorded prior to treatment, at 2 and 4 weeks post-treatment. The mean defect of wounds treated was 19.21 cm3. Our results showed a statistically significant reduction in wound size of 45% and 59% at 2 and 4 weeks post-treatment, respectively (p < 0.01). ADM was also effective in infected DFW as 84% of our wounds had positive tissue cultures at the time of application. Micronized injectable ADM has proven to be an effective treatment for DFW. Advantages include a ready-to-use injectable, single-stage treatment, minimal pain, mouldable matrix to fit any wound shape, allows for outpatient treatment and simple wound dressings.
This meta-analysis study aimed to assess the effectiveness of acupuncture in enhancing wound recovery in patients experiencing leukopenia following chemotherapy for malignant tumours. A comprehensive search was conducted to collect randomized controlled trials and observational studies focusing on leukopenia post-chemotherapy in malignant tumour patients. Data analysis was performed using Revman 5.3 software. Eleven articles were included, encompassing 957 patients. The meta-analysis revealed that, compared with control groups, patients receiving acupuncture showed a significantly higher total effectiveness rate (p < 0.05). Subgroup analysis indicated superior outcomes in groups receiving acupuncture combined with moxibustion (p < 0.05) and acupuncture combined with Western medicine (p < 0.05), with the former outperforming the latter. Notably, acupuncture was found to improve white blood cell (WBC) counts on 3rd, 7th, 14th, 21st and 28th days post-treatment, with significant differences compared with control groups. These improvements were time dependent and more pronounced than in control groups. Acupuncture demonstrated significant, time-dependent capability to enhance wound recovery in leukopenia following chemotherapy for malignant tumours. The combination of acupuncture and moxibustion was found to be more effective than acupuncture combined with Western medicine. This study highlighted the potential of acupuncture as a complementary therapy in managing chemotherapy-induced leukopenia, contributing positively to the wound healing process in cancer patients.
