Use of oxygen-ozone therapy to improve the effectiveness of antibiotic treatment on infected arthroplasty: protocol for a superiority, open-label, multicentre, randomised, parallel trial

Introduction

Surgical site infections still remain a major public health challenge and have become an increasing universal risk, especially for the implantation of orthopaedic devices.

Unfortunately, the discovery and increasingly widespread use (especially the misuse) of antibiotics have led to the rapid appearance of antibiotic-resistant strains today; more and more infections are caused by microorganisms that fail to respond to conventional treatments.

Oxygen-ozone therapy has been extensively used and studied for decades across various potential medical applications and has provided consistent effects with minimal side effects.

This study aims to determine the superiority of oxygen-ozone therapy in combination with oral antibiotic therapy in patients with wound infections after an orthopaedic device implantation when compared with antibiotic therapy alone.

Methods and analysis

This is an open-label, multicentre, randomised, parallel-group study that aims to assess the efficacy and safety of oxygen-ozone therapy in combination with oral antibiotic therapy to treat infections in patients (male or female aged ≥18 years) having undergone surgery for the implant of an orthopaedic device. Patients must have at least one (but no more than three) postoperative wounds in the site of surgery (ulcers, eschars and sores) and at least one symptom (pain, burning, redness and malodour) and at least one sign (erythema, local warmth, swelling and purulent secretion) of infection of at least moderate intensity (score ≥2) in the target lesion at the screening visit (patients with wounds without signs of localised infection or with undermining wounds will be excluded).

Patients (n=186) will be recruited from five Italian hospitals and studied for 7 weeks. All will be assigned to one of the two treatment groups according to a web-based, centralised randomisation procedure and placed into either the (1) intervention: oxygen-ozone therapy 2–3 times a week for 6 weeks (for a maximum of 15 sessions) simultaneously with an appropriate oral antibiotic therapy prescribed at baseline or (2) control: oral antibiotic therapy prescribed at baseline.

The primary outcome is the efficacy and superiority of the treatment (ozone and oral antibiotic therapies); secondary outcomes include the resolution of signs and symptoms, modifications in lesion size and the treatment’s safety and tolerability.

Ethics and dissemination

This study has been reviewed and approved by the responsible Independent Ethics Committee (IEC) of COMITATO ETICO CAMPANIA NORD, located at ‘Azienda Ospedaliera San Giuseppe Moscati di Avellino’.

After completion of the study, the project coordinator will prepare a draft manuscript containing the final results of the study on the basis of the statistical analysis. The manuscript will be derived by the co-authors for comments, and after revision, it will be sent to a major scientific journal. Findings will be disseminated via online and print media, events and peer-reviewed journals.

Are pharmacists on the front lines of the opioid epidemic? A cross-sectional study of the practices and competencies of community and hospital pharmacists in Punjab, Pakistan

Introduction

Countries are grappling with a rapidly worsening upsurge in the opioid-related overdose deaths, misuse and abuse. There is a dearth of data in Pakistan regarding the practices and competencies of pharmacists in handling opioid-related issues.

Study design

A cross-sectional study, conducted across Punjab, Pakistan.

Method

The study deployed a validated survey to evaluate the competencies and practices of the community and hospital pharmacists.

Results

504 community pharmacists and 279 hospital pharmacists participated in the survey with an overall response rate of 85.5%. Almost half of the respondents ‘never’ or ‘sometimes’ made clinical notes in a journal or dispensing software to monitor ongoing opioid use. Generally, pharmacists were reluctant to collaborate with physicians or notify police regarding the abuse/misuse of opioids. Conclusion

Both community and hospital pharmacists hold significant positions and potential to contribute meaningfully to the mitigation of harms and risks associated with opioids. Nevertheless, this study underscores notable deficiencies in the competence of pharmacists, whether in hospital or community settings in Punjab, concerning various aspects related to the dispensing and utilisation of opioids. It also highlights the pressing need for the development of strategies aimed at improving several practice areas including the documentation, the quality of patient counselling, the effectiveness of reporting mechanisms for opioid abuse and the stringent enforcement of regulatory policies to curtail opioid misuse. Thus, to mitigate the opioid epidemic in Pakistan, it is imperative to institute opioid stewardship initiatives aimed at rectifying the competency and procedural deficiencies within the pharmacist workforce.

Objetivo principal: Con este texto biográfico, nos aproximamos a las vivencias de un músico profesional, una contrabajista, tras un proceso crónico de dolor de hombro, muy prolongado en el tiempo, que resultó en dos intervenciones quirúrgicas. El objetivo principal que se planteó fue conocer las percepciones de la paciente respecto al proceso y sus relaciones con los profesionales sanitarios, así como la opinión derivada de la asistencia recibida. Metodología: La información se obtuvo por medio de una metodología cualitativa realizando una entrevista en profundidad.  Resultados principales: La informante relató su experiencia desde la aparición de las primeras molestias, centrándose principalmente en la compleja interacción que ha experimentado con los servicios sanitarios, así como las implicaciones emocionales y laborales de esta patología. Conclusión principal: Destaca su personalidad y pensamiento optimista, así como su capacidad de reinvención y superación de las dificultades, cobrando especial relevancia el apoyo constante del entorno familiar y profesional.

Valoración de las infecciones del tracto urinario en ancianos institucionalizados. Propuesta de mejora en el abordaje clínico

Objetivo: analizar las ITU en una Residencia y diseñar una estrategia de manejo y prevención. Metodología: estudio descriptivo, retrospectivo y longitudinal de ancianos institucionalizados en una Residencia de Cádiz entre el 1/abril/2015 al 31/marzo/2016. Seleccionamos aquellos que padecieron al menos un episodio de ITU. No presentamos criterios de exclusión. Resultados: contamos con 119 residentes, (media de 88/mes), presentaron infección 44 (75% mujeres). Edad entre 71 y 102 años, moda y mediana de 84. Tasa de incidencia global 49%/año. Sondaje vesical un 15,90% e incontinencia urinaria 86,36%. El 79.1% fueron episodios agudos, 20.9% crónicos y el 68.08% recidivaron. Manifestaciones clínicas: fiebre, astenia, dolor, hiperglucemia y hematuria. Escherichia coli. como germen más habitual. Conclusión: constatamos alta prevalencia de ITU mejorable con estrategia de gestión multidisciplinar, evitando sobrediagnóstico y sobretratamiento y prevención primaria con probióticos.
